Doctors who don't wash their hands (pain, infection, skin, dentist)

What do you do when a doctor does not wash their hands or use hand sanitizer? I have requested they do 3 times when they never did and I felt they were extra rough when doing the exam and caused me pain as retaliation. But I have gotten a staph infection from a doctor that did not wash their hands. What do you do?
How do you know they did not? Depending on their office layout they may use sanitizer just outside of the room?
Also, I have doubts that they intentionally caused you pain in retaliation. How did you confirm that the staph infection was from THIS doctor? If you really believe this, you're silly to see them again but you need to be sure of what you're concluding.

Proper hand hygiene for the health field is that the patient sees you washing your hands or sanitizing them.
Trust me, there are plenty of nurses and doctors that do not practice proper hand hygiene and they are called out on it.
Okay...where do you live? I've never seen such a concentration of "dirty doctors" - I suggest you move to a major metropolitan area that has actual running water
We also require that they sanitize the stethoscope in front of us. They had some skin thing they spread where we lived!
I was in a waiting room at the hospital waiting for our son to be called in for 45 minutes. During that time, the monitors that lets you know it was your turn was given to several different people without being sanitized. I watched a toddler suck on one for quite some time. I addressed this and got it changed.
We primarily avoid the big medical machine, a best practice when it comes to maintaining one's health.
A few years ago I had a horrible toothache and went to the first dentist I could find. The dentist was some mad scientist looking guy who was like a month from retirement and did not wear gloves. I was in too much pain to care.
