    Violation from previous inspection has been corrected by beer keg drain line being rerouted to floor sink. Another drain line was found draining to this pan but was fixed during inspection. Nothing should drain to this pan as the pan is not flat and any water in the pan ends up pooling to one side and not draining properly.

    Observed beer keg drain lines pooling in pan under dishwasher. Drain lines must terminate into a floor drain with a proper air gap to prevent pooling of standing water and beer.

    All the following violations from previous inspection have been corrected. - Both dishwashers are dispensing 100 ppm Chlorine. All glasses in bar must be rewashed and sanitizer logs must be kept per internal Boomtown policy; at a minimum of twice per shift. - Tequila bottle has been replaced in liquor dispensing system. - Ice machine has been cleaned. - Hand sinks have been stocked with soap and paper towels. - Racks have been removed from floor in back and floors have been cleaned.

Dishwasher must be able to achieve sanitation without being primed each time.Server station dishwasher: 0ppm chlorineSanitation logs are not being maintained per requirements instituted by Inspector from 2014 inspection. System developed by management to check every 2 hours is not being followed by employees/bartenders..

    Sanitation buckets 200ppmConsumer ice (food ice) stored separate from ice used for chilling/cooling.The following violations must be corrected within 48 hours:1) Repair dishwashers to allow proper sanitation of glasses. Dishwashers were not repaired by end of inspection and the bar must switch to single-use; disposible cups and discontinue service in glassware. When the dishwashers come back into service; every piece of glassware in the bar must be rewashed before being placed back into service and a log maintained every 2 hours showing the concentration of sanitizer until reinspection.2) Replace tequila bottle or cover hole in dispensing system.3) Discard ice in the machine and wash and sanitize the walls and deflector plate before putting the unit back into service.4) Removing shelving in contact with the floor and sweep and mop under the shelving units in back store room.**Soap dispenser and drain lines corrected on site.Some of these violations are repeat violations from 2014. If the above violations are not corrected by Monday; February 2; 2015; and the Inspector is not contacted for an extension; reinspection fees will apply for subsequent reinspections.

    Observed both dishwashers draining into large metal plates and thus creating standing pools of water. These units need to drain into nearby floor sinks or directly into the drain within the metal plate (while still maintaining an air gap) to eliminate any standing water.*** Note: This item has been noted on two previous inspections (7/31/14 & 8/6/14) and discussed with management staff during other inspections and still have not been addressed. If parts need to be ordered to address this issue then WCHD staff needs to be informed of steps being taken to abate this issue. No attempt to inform WCHD has been made to date. A reinspection fee of $71 will be assessed in order to confirm this item has been corrected.

    All violations from 7/31/14 inspection with the exception of the above have been corrected: - Consumer ice has been separated from speed gun lines. - Ice scoops have been removed from unused ice bin. - Both dishwashers are dispensing 50 ppm Chlorine. Sanitizer logs are being kept daily. - Facility is much cleaner. * Ensure areas stay clean. - Dump sink at waitress station is draining into floor sink now.- Drain for hand sink is now terminating directly into floor sink. - Metal plate under blender has been replaced.

    Observed speed gun lines running through consumer ice. These lines may be ran through ice to keep them cold but consumer ice must be kept in a self draining container separate from ice surrounding speed gun lines. Corrected on site.

    Observed ice scoops stored in unused ice bin that is dusty and dirty. Ice scoops should be stored in a clean and sanitary manner at all times to prevent cross contamination.

    Observed both dishwashers not dispensing sanitizer. Facility has test strips on hand but they are only utilized once per week. Solution changed at main dishwasher and machine primed and achieved 50 ppm Chlorine. Unit at waitress station was not able to be fixed during inspection therefore this unit is placed out of service. Unit should be serviced and may not be utilized until reinspected by WCHD staff.Facility must keep sanitizer logs for both dishwashers until further notice. Log sheet examples supplied to operator.

Drains in metal pans will then need to be sealed off or periodically flushed to keep sewer traps full of water.

    Observed metal plate under blender that has been soldered on with large gaps that can harbor bacteria and are not easily cleanable. This metal plate should be removed and replaced; if necessary; with a surface that is smooth and easily cleanable.

    Observed various non food contact surfaces dirty. Entire facility needs a thorough cleaning to prevent cross contamination and prevent attraction of vermin.

This sink should draing into nearby floor sink.Observed drain for hand sink not terminating directly into floor sink. Water is hitting tile nearby and then eventually into the floor sink. This drain needs to terminate directly into floor sink in order to eliminate standing water.

    Hand sink clean and stocked. *Soap and paper towel dispensers should be permanently mounted near hand sink.Speed guns clean. Test strips available.*Replace or resurface garnish cutting board as it's beginning to develop deep groves and stains.Ice machine is currently out of service for repairs. Ice is being supplied by main kitchen and stored in large plastic tubs with lids. No issues.
