Received a complaint on 8/10/2015 that the men's room had no faucet for the hand sink. I spoke with the manager and she said someone broke the handle off the faucet on Monday (8/10/2015). Received a complaint that trash around the dumpster has not been picked up and it's attracting rodents. Observed a lot of trash around the compacting dumpster. Manager stated that it has been broken for over a week and the company that provides and maintains it brought another dumpster out until they can repair the compactor. Manager has called the company several times about repairing the compactor and they told her they are waiting for parts. She called the company again while I was there and scheduled an extra pick up for today to get all the trash outside the compactor. They will clean the area around the compactor once the trash is removed. No violation noted during this evaluation.	07/08/2015	Complaint
Quat sanitizer concentration in 3-vat sink measured at 400 ppm. Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Canadian bacon in 2-door under counter cooler was at improper temperatures. Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Equipment - Good Repair and Proper Adjustment Observation: Two of the three drain plugs for the 3-vat sink will not hold water. Correction: Replace the damaged drain plugs and insure the 3-vat sink can be set up properly. Correct today. Physical Facilities in Good Repair Observation: Wall behind dry storage shelves is in disrepair. Correction: Maintain physical facilities in good repair. Received a complaint that there are roaches in the electrical panel, uncovered food in the freezer and coolers, open food containers on the floor, the freezer roof is leaking, and there is mold on the fire extinguisher. I conducted a routine inspection of the establishment and did not observe any roaches or evidence of roaches in the establishment. The only uncovered foods in refrigeration units were some pies in the walk=in freezer. There is some ice on the floor and ceiling of the walk-in indicating a possible problem. Instructed manager to defrost the freezer and insure it is functioning properly. I did not observe any open food containers on the floor. Received a complaint that a customer had bugs in his drink and there were bugs around the soda machine. I conducted a routine inspection of the facility and during my inspection I only found one drain fly in this area. I spoke with the manager and she said after the customer complained to them they cleaned the area around the soda machines. This area as well as the beverage dispensing nozzles are cleaned every night according to management. Ecolab comes out regularly to check for pests and there is an enclosed fly light in between the soda machines. I did observe several drain flies in the kitchen and several dirty, stopped-up drains (in the area of the 3-vat sink). Discussed my observations with the manager and they will be cleaning out the drains. Manager also stated she called Ecolab on 9/11/2013 to come out in between visits to help control for the drain flies. No violation noted during this evaluation.	09/12/2013	Complaint
Quat sanitizer concentration in wiping cloth bucket measured at 150 ppm. Critical: Time as a Public Health Control* (corrected on site) Observation: Sliced tomatoes for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time. Correction: Discard food that is not cooked and served or served if ready-to-eat within 4 hours from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time. (Correction: Sliced tomatoes were marked with a 2-hour use or discard by time with stickers on the container- manager will continue use of stickers until a new clock hand board is delivered.) Warewashing Sinks, Use Limitation (corrected on site) Observation: The compartment(s) of the manual warewashing sink was observed used for handwashing activity. Correction: A warewashing sink may not be used for handwashing or dumping mop water due to risk of exposure of food or food contact equipment and utensils to contamination. Correction: instructed employees to use one of the designated handwashing facilities to wash their hands. Handwashing - Using a Handwashing Lavatory Observation: The handwashing facility located near the 3-vat sink is leaking and the drain is stopped up, preventing use by employees for easy handwashing. Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Repair the leak and unclog the drain so the sink is useable.Correct by 9-15-2013. Pests - Controlling Pests* Observation: Observed some drain flies in the back of the kitchen (mainly around the 3-vat sink). There are several dirty drains with some standing water in this area.
