This visit was made to conduct a routine food inspection. The following items require correction.
-Please label foods packaged with items made in the store with a label containing the Name, Contact information, Allergens, and Net Weight of the package. Bulk foods must also be labeled with manufacturer/processor label and a common name.
*Repeat violations subject to civil penalties.

  • Food Labels/Common Name/Ingredients/Quantity/Manufacturer
    Observation: Insufficient information is provided on the label of the following food item(s) packaged in the food establishment: Cheeses, Soaked Nuts, Sweets, bulk foods
    Correction: Food packaged in a food establishment for customer self service shall include labels that contain the following information: 1. the common name of the food, 2. the list of major ingredients including artificial colors or preservatives, 3. the total net weight or quantity of the food, 4. the name and place of business of the food manufacturer, packer, or distributor, 5. the name of the food source for each major food allergen contained in the food,6. nutrition labeling and, 7. for any salmonid fish containing canthaxanthin as a color additive, the labeling of the bulk fish container, including a list of ingredients, displayed on the retail container or by other written means.

This visit was made to conduct a routine food safety evaluation. Owner was very helpful and knowledgeable on employee health policy. The following items require correction:
1.

  • Package Integrity & No Dented Cans/ Specifications for Receiving (corrected on site)
    Observation: Bulk #10 cans dented and offered for consumer retail sale.
    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit. Owner will not offer dented cans for sale, discussed not accepting damaged cans from distributor.
  • Critical: Food on Display Protected From Contamination
    Observation: The food on display is not protected from contamination. Dried fruit and dried beans stored in large bins for self-dispensing not protected with a sneeze guard or shield.
    Correction: Food on display shall be protected from contamination by the use of packaging, counter, service line, or salad bar food guards, display cases, or other effective means.

    Observation: Insufficient information is provided on the label of the following food items packaged in the food establishment: bulk food items (dried beans and spices).

    Observation: Light fixtures in upright refrigerators are not working (burnt out bulbs).
    Correction: Replace light bulbs to provide sufficient lighting..

02/24/2014Risk Factor
The purpose of this inspection to determine if the establishment was sufficiently cleaned to open after the store was flooded on 1/28/13. The following deficiencies must be corrected in 10 days.
1. The storage racks in the walk-in units, the merchandise storage shelves in the store, reach-in refrigerator unused for storage all needs cleaning.
2. Replace all missing and damaged ceiling tiles in the store and rest-room.
3. Provide clean cloth dispensing towels in the rest-room. Correct immediately.
4. Discard single service container improperly stored on the floor.
5. Provide light shield in the rear kitchen area..
6. Repair the floor tiles in the store (near the front window) and cracked floor tiles in the meat department.
7 .Sanitizer spray bottles are improperly stored with clean equipment. Corrected during follow-up inspection.
8. Remove all unnecessary and unused equipment: ice machine and two freezer chest in the meat department.
9 Replace missing wall molding in the restaurant area
The store is approved to open

The purpose of this inspection to determine if the establishment was sufficiently cleaned to open after the store was flooded on 1/28/13. The following deficiencies must be corrected:
1. Employee's coat and hat are improperly placed on top of soft drinks.
2. Face plates are missing in the meat department, and in the restaurant.
3. The storage racks in the walk-in units, bulk containers in the walk-in unit, the merchandise storage shelves in the store, the cutting boards in the restaurant and the warmer unit in the restaurant, the reach-in refrigerator unused for storage and the cooking equipment all needs cleaning.
4. Replace all missing and damaged ceiling tiles in the store and restaurant areas.
5. Remove peg board in the restaurant area. Provide smooth and easy to clean surface.
6. Provide soap, paper towels or clean cloth dispensing towels at both hand sinks in the restaurant and in the rest-room.
7. Discard single service container improperly stored on the floor and dusty pizza boxes.
8. Provide light shield in the rear kitchen area and the drop light at the front counter area in the restaurant.
9. Replace light bulbs that area out in the rear kitchen area and at the front counter area in the restaurant.
10. Clean the floor in the store and the restaurant.
11.Repair the floor tiles in the store (near the front window) and cracked floor tiles in the meat department.
12. A detergent container is improperly used to store kabob skewers. Sanitizer spray bottles are improperly stored with clean equipment.
13. Remove all unnecessary and unused equipment: ice machine and two freezer chest in the meat department.
14 Replace missing wall molding in the restaurant area

Pay the health department permit fee immediately. The establishment can not operate without a current permit. All food must be protected from contamination. Provide a scoop for each item sold in the bulk bins. Training manual issued. Repeat violations are subject to fines.
