- Observed rusty shelves in the walk in cooler. Repaint or spray.
- Observed broken gaskets on the doors of the display part of the walk in cooler. Replace. Observed tape being used to hold material blocking cup dispenser under soda machine. Remove tape.
- Observed wiping cloths spread on 3 compartment sink. Discard for washing. Keep wiping cloths in sanitizer solution between use.
- Observed accumulation of ice and dirt in the white cooler in the back storage area. Defrost and clean. Observed debris build up in the following areas/equipment: shelves in walk-in cooler/soda display shelves, door of soda display (inside), soda machines (areas around the nozzles, microwave under cappuccino machine, inside of the donut display, and the inside of all the display coolers in the general area - including the door gaskets and the inner frame of equipment. Clean.
- Observed debris in the following areas: shelving units holding soda in the back storage area, cabinet (inside) under soda machine/donut display (evidence of gnawing activity of mice and feces observed), top of shelves in general display area - including top of cabinet holding beverage dispensing machines and donut display unit holding beverage dispensing machines, and the donut display unit.
- Observed micro-filtration devices stored in the hand sink in back storage area. Items were removed during inspection. Keep hand sink accessible.
- Observed objectionable odor and debris on hand sink and towel dispenser in bathroom. Clean
- Observed mice feces scattered throughout facility, especially in the corners and under storage shelves in the back storage area, in cabinets in the general display area, and under the 3 compartment sink in the back storage area. Per manager, there was an infestation and it had been for a couple of months. Manager said they had a pest control company come out once a month to set/check traps. Advised manager to clean entire facility, making sure that all mice feces are cleaned out. Facility must be cleaned within 24 hours of this inspection. Inspector will be back to inspect at 10 am, 12-4-2015. Inspector also advised facility to intensify efforts to eradicate mice in facility.
- Observed dirt on floor throughout facility, including corners, under and around equipment and shelves (especially in the back storage area). Observed debris on floor of walk in cooler and a rag under the shelf in the cooler. Clean floors.
- Observed missing floor tiles in the back storage area (by the entrance to the display area. Replace.
- Observed clutter throughout facility, especially the back storage area, and storage area behind the counter. Remove clutter.

- In the walk in cooler there were opened hot dogs with no date label. Per manager she takes them from the freezer, opens them and put them in the walk in cooler, then puts them on the hot dog roller. Explained that once you open the plastic package out of the freezer you must label with the date.
- The manager couldn't locate the metal stemmed thermometer.
- At time of inspection, the tongs for the donuts and hot dogs were sitting out despite not having either. Explained not only they should be taken to the back when not in use, but also washed, rinsed and sanitized every 4 hours when in use. When you do have donuts and hot dogs the tongs should be covered.
- The hot dogs were stored in a storage container that didn't appear to be food grade.
- The cabinets under the soda fountain are broken and appear to have water damage. There are also shelves that have exposed wood. There are broken seals on the soda cooler. In the back there was cardboard lining a shelf.
- The facility didn't have any chlorine test strips.
- The cabinets are dirty under the soda fountain. There is black buildup on the soda cooler.
- The faucet on the 3-compartment sink leaks. There was also water accumulated on the ground under the mop sink. I suspect something is leaking.
- Observed gap around door to outside. Door must be repaired so no daylight is visible. Please repair by 7-22-15.
- There are damaged floor tiles.
- The floor is dirty throughout the facility, especially in the walk in cooler and in the back area.
- There are stained ceiling tiles. In the walk in cooler there is dust accumulation in the fans and on the ceiling. There is a hole in the ceiling in the back by the walk in cooler door.
- In the shopping area there was Ramen stored next to brake fluid. Also there was an opened box of gloves stored next to Windex and toilet cleaner. Also there was Raid stored by cleaners. The gloves and Raid were removed. The manager moved Ramen away from the brake fluid and will rearrange things.
- There is old equipment in the back area including an upright refrigerator/freezer unit, ice machine, and printer. You need to go through the items in the back and get rid of extraneous items.
- The only food manager's license posted was expired. As a category 2 you should have someone with their food manager's license that works here. Not someone who works at another store. I will email a schedule of classes at TCHD.
