- Found container of raw sausage stored above R-T-E tomato sauce in lower reachin prep cooler. Food Mgr moved raw sausage to bottom shelf of cooler.
- No handle on food dispenser for semolina mix or for aujur sauce.
- One employee did not have on an effective hair restrain.
- No chem sanitizer PPM in dishwasher. Use 3 comp sink until fixed.
- Found two moist wiping clothes stored on counters.
- Men's restroom door was open. It is not self-closing.

- Observed tape on the salad bar. Remove; Observed broken gasket on the Pepsi and prep cooler door. Replace. Observed rusty shelves in prep cooler. Repaint.
- Observed accumulation of debris on/in the dish washing machine. Clean.
- There were no test kits for testing sanitizer concentration in the dishwasher. make appropriate test strips available for corresponding sanitizer in dish washing machine.
- There were no test kits for testing sanitizer concentration in the 3 compartment sink. Make appropriate test strip available for corresponding sanitizer in use.
- There was no indication of sanitizer in machine at the time of inspection. Ensure dispensation of sanitizer in machine and appropriate concentration when in use. Repair and inform TCHD.
- Observed debris build up on dish washer racks/crates. Clean. Observed debris build up on edges of dough trays. Clean. The walk-in cooler door and shelves have debris build up. There was also build up in the beverage cooler in the service area, inside the prep unit cooler especially the door gasket, inner frame and corners of hinges on the upper part of unit. Clean. Observed food debris on pizza oven. Clean. Observed debris accumulation on knives stored by the prep table in kitchen. Clean, including the handles. Observed debris on prep tables. Clean.
- Observed debris accumulation on the gray cart in kitchen. Clean. Outside of the walk in cooler door has debris build up especially the handle area, outside of the beverage cooler out front also has debris build up. Clean. Observed debris on shelving units in dish washing area. Clean.
- Observed broken floor tile under prep unit in kitchen. Replace.
- Observed debris build up behind and around equipment in kitchen, especially behind the pizza oven, under the hand sink and in corners by the 3 compartment sink and walk-in cooler. Clean floor.
- Wall behind prep table in kitchen and knife storage area, as well as the 3 compartment sink/dish machine area have build up of debris. Clean walls.
- Sanitizer concentration in spray bottle was above 200 ppm chlorine. Staff discarded sanitizer during inspection, they said they used wiping cloth in sanitizer solution instead of the spray bottle.

- Observed the use of drinking cup used as scoop for shredded cheese in the cold holding and a bowl used to dispense semolina. Use proper scoop that will not allow handle to come in contact with food.
- Observed employees not wearing effective hair restraints while doing food prep.
- Observed exposed wood surface used as shelving below the hot holding counter as well as in the basement. Also observed a damaged counter tile in-front of hot holding.
- Found build-up on can opener blade. Observed food debris build-up in pizza oven
- Observed gap on the bottom left and in the corner to outside across from the basement stairs. Door must be repaired so no daylight is visible. Repair by 4-17-15. Contact Katie once the door is repaired.
- Food damaged/missing floor tiles.
- Found food splatter and dough on walls in kitchen.
- Not all employees have gotten their food handlers licenses. Please create a chart that says the employee's name, date of hire, and date of training.
