- Observed employee incorrectly wash hands. Employee turned off faucet with wet, bare hands. Ensure employees use a disposable paper towel to turn off faucet. Demonstrated proper handwashing for employees.
- Observed heavy dust and grease buildup on vents above wok station and fryers. Ensure vents are removed and cleaned on a regular basis.
- Handle bar at prep top cooler across wok area is broken. Repair. Also observed tape being used to hold cooler gaskets together. Remove duct tape then repair or replace damaged gaskets.
- After observing heavily soiled can opener, instructed employee to re-wash, rinse, and sanitize it. Employee washed and rinsed can opener, then set it to dry. Instructed employee this needs to be sanitized for at least 60 seconds. Employee set can opener in sanitizing solution.
- The following areas have food and grease buildup; handle bars of reach in coolers across wok station, both rice warmers in kitchen, and
- Observed one live cockroach on wall along kitchen ice machine and one running on floor along buffet line. Observed dead roaches in the following areas; in a sanitizer bucket in the kitchen, underneath wok station, on kitchen ceiling light shield, and walk in cooler. Per pest control receipt manager showed, last time pest control inspected was in December. A report was not given to facility by the pest control people indicating what was observed or treated. Facility must contact a licensed pest control and treat for appropriate pests. A re-inspection will be done after 10 days, 2-29-2016, to observe what effective measures are being done to control pests.
- Observed food debris and grease buildup on floors in kitchen, especially near wok station, cookline coolers, and in walk in cooler. Clean floors.
- Observed grease and debris accumulation on the walls near the fryer and on the fan in the walk in cooler. Clean walls.

- The following food items were at 48 F in the dining room cooler: One gallon of milk, One container of cottage cheese, One container of pudding, and one container of ranch. Per the owner, he had turned the cooler down several days ago and has not checked the temperature. These items were voluntarily discarded at the time of this inspection. No potentially hazardous food items may be stored in this cooler until TCHD has verified that the cooler is maintaining temperature at a minimum of 135 F. Contact this department for re-inspection.
- Observed 2 trays of fish that per the owner, were thawing on racks in the kitchen. The fish was below 41 F. The trays of fish were moved to the walk-in cooler at the time of this inspection.
- Observed raw beef stored above cabbage. Observed raw chicken stored above raw beef. To prevent contamination, raw meat should not be stored above ready to eat foods and raw meat that has to reach a higher minimum internal temperature must be stored below raw meat that has to reach a lower minimum internal temperature. These items were rearranged properly in the cooler at the time of this inspection.
- Observed two open employee beverages in kitchen. Cups must be lidded or enclosed. These beverages were voluntarily discarded by owner at the time of this inspection.
- Observed kitchen staff without hair restraints. Ensure effective hair restraints are worn.
- Observed food items stored in plastic grocery bags. Food items must be stored in food grade containers.
- Observed tape used to seal the white chest freezer in basement and along the seals in the prep cooler in the kitchen. Remove and replace seals.
- Observed wiping cloths stored on counter tops throughout kitchen. Ensure wiping cloths are stored in sanitizer solution when not in use.
- Observed debris accumulation on and in the refrigerator that is across from the fryer and in the cabinets beneath the soda machine. Clean and ensure routine cleaning.
- Observed cups that were stacked wet. Dishes must be air dried before storing.
- Observed chipping and peeling paint on the ceiling in the storage area above the stairs. Repaint.
- Observed debris accumulation on the wall in the basement storage room as discussed with the owner during this inspection. Observed debris accumulation on the walls near the fryer and on the fan in the walk in cooler. Clean and ensure routine cleaning.
