- Observed employee working at the 3-compartment, cleaning with a wet rag, then put on gloves and do food prep. I didn't see her wash her hands once. Since the last inspection the manager did conduct handwashing training. Discussed not only going over how to properly wash hands, but also when to wash hands. I need documentation of this by 7-10-15. I gave a norovirus handout to go over as part of training.
- In the kitchen on the prep counter there was a cup of rice with a fork in it. The facility doesn't serve rice. The cup was removed when the employee left. You need to make this a part of the training- only eating in designated areas.
- Employee doing food prep in the kitchen didn't have any hair restraint.
- Observed containers being used to hold food that didn't appear to be food grade.
- The paint on the walk in cooler door is coming off. The racks in the reach in sandwich cooler has paint missing which has rusty metal. The shelves under hot holding units has exposed wood. There is a crack in the counter that holds coffee.
- Under the coffee station, in the cabinets, there is an accumulation of debris and coffee grounds. The icee area is very sticky on cabinets.
- In the sandwich area there is a trash can blocking the hand sink. The manager moved it. This should be included in the training.
- Under the oven there are damaged floor tiles.
- Under the ice machine, the floor has debris and buildup.
- There is a light in the kitchen, over the dough roller, that isn't shielded.
- Observed the mop being stored in dirty water.

- Observed employee improperly washing their hands. The employee turned off the faucet with her hands instead of using a paper towel. You need to train all employees on how to properly wash their hands, emphasizing using the paper towel to turn off the faucet. Once you have trained all employees you need to watch every employee properly wash their hands to verify they are able to correctly. I need documentation of training and that you watched every employee wash their hands by 2-4-15.
- The lid of the pizza prep cooler has tape on it because it is falling apart. Per manager she has ordered another.
- Found build-up on handles of prep coolers. The cabinet under soda fountain had some sort of buildup or stain.
- Handsinks were not accessible. The handsink behind the sandwich prep had a trash can in front of it and the handsink by the ice machine had a wine box in front of it. You can not have anything in or around handsinks. The manager has put up signs at handsinks telling employees not to block handsinks. Discussed moving the garbage away from the handsink so it can't be pulled out and block the sink.

- Facility's metal stemmed thermometer was not able to be located.
- The large stand mixer in the kitchen has paint coming off. Also paint coming off walk in cooler door.
- The employee in the kitchen didn't know where the test strips were.
- Found build-up on handles of prep coolers. The cabinet under soda fountain had some sort of buildup or stain. Also the bottom of utensil drawer had crumbs and food buildup.
- On the sandwich hand sink there was a can placed on it. Also at the ice machine hand sink there was a scoop and ice bag in the sink and a dolly in front of sink. Discussed with manager that hand sinks must be accessible at all times. You can not have anything in or around them. The items were all removed.
- It appeared as if soda or something sticky had spilled in walk in cooler
- In the kitchen, there was mouse/rat poison pellets under the 3-compartment sink. Discussed with the manager that these are not approved for food facilities. It is possible for the poison to contaminate food or food prep areas. The mouse poison was removed. In the future, either consult with a licensed pest professional or put down traps that do not have poison.
- On the shelf by the ice machine there were multiple bottles of bleach stored over an ice scoop. It was not the ice scoop that was currently in use. Discussed with manager that either the bleach needs to be moved down a shelf or the extra ice scoops should be stored somewhere else.
