- Observed torn cooler gaskets near Redbull and energy drink. Replace damaged door gaskets.
- Men and women restroom doors are not self closing. Install a self closing latch so doors remain closed at all times.

- There are open boxes in the walk in freezer in the kitchen.
- The arms of the stand mixer are rusted. The hand sink by that mixer is coming away from the wall. There are paper liners on chemical storage trays. There was tape on beer cooler handle. (It was removed). The dryer is rusted at bottom. There is laminate missing on the cabinet under the microwave.
- There was a beer can and bag in the hand sink by ice machine. Also cans of alcohol and soda in front. The can and bag was removed from the sink. The cans are at least to the side so it's not blocking it completely. Discussed making sure all hand sinks are accessible. Ice is considered a food so bagging it is food prep and must have a hand sink.
- There are mop heads on the floor under the pizza prep cooler and the sandwich hand sink.
- Coving coming away from wall by ice machine.
- The floor needs swept outside and inside the walk in freezer.

- Found bag-in-box soda stored on the floor in the back hallway. Also, found food on the floor of the walk in cooler (from the North Pekin store remodel).
- The facility was using chlorine sanitizer but did not have the proper test strips. Provide chlorine test strips.
- The sandwich oven had food debris build up. Clean it.
- Found build-up of debris on floors. Clean the floors.
- Found chlorine sanitizer in the wiping pail at the front counter at >200 ppm. Corrected to 100 ppm.

- Box of grated cheese in the walk in cooler opened with no label of when it was received or opened.
- Observed kitchen employee wash hands and then turn the water off with her bare hand. When turning the water off, use a single use paper towel to touch the faucet. Suggested to manager to do a retraining with all employee on proper hand washing technique and glove use hygiene. Submit documentation of training performed and employees that attended.
- Manager did not have hair restraint. Other employee walked into kitchen without hair restraint as well. All employees in the food prep area/kitchen need to wear effective hair restraints.
- Found ladles stored in drawer with chipping metal. Replace the ladles.
- Cooler racks in the prep area near the walk in cooler are rusting. Repaint the racks. Sealant behind handwashing sink in pizza oven area is chipping. Reseal.
- All food contact surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include the soda machine grate, the coffee and creamer nozzles and grates.
- Found black build up on racks in the walk in cooler near the cans of energy drinks. All racks need to be washed, rinsed, and sanitized. Include cleaning racks to schedule of routine cleaning of facility.
- Plastic containers stored wet above the 3 compartment sink. Air dry all dishes completely before storage.
- Food missing floor tile under the 3 compartment sink.
- Build up on the floor by the door between the walk in cooler and the liquor cooler. Clean the floors.
- Found unlabeled chemical container. Label all chemicals not in their original container.
- Found chemicals and pesticides stored next to each other on a rack beneath the 3 compartment sink. These chemicals need to be physically separated from each other. Manager moved the pesticide completely out of the kitchen.
