- Observed employee make burger for customer by grabbing raw meat with deli tissue paper, bun with tongs, and pickles with tongs. Employee used bare hands to grab onions and cheese. Discussed with employee that she must use gloves or a utensil to grab any ready-to-eat foods. Employee discarded cheese and onions, and wore gloves to grab new food.
- Observed employee not wearing effective hair restraints when handling food. All employees that handle food must wear effective hair restraints (hat or hairnets) to prevent contamination.
- Observed damage to shelving in walk-in refrigerator in bar area. Food contact surfaces must be smooth, easily cleanable, free of chips/breaks/abrasions, and maintained in good repair. Repair or replace to allow for proper cleaning.
- Observed towel under utensils in the cooking area. All food contact surfaces must be smooth, easily cleanable, and non-absorbent. Towels create a surface that is absorbent and not easily cleanable. Remove paper towels from food contact surfaces.
- Observed exposed, untreated wood on floor in back storage area and in walk-in refrigerator. Untreated wood is absorbent and does not allow for proper cleaning. Paint, reseal, or replace to ensure proper cleaning.
- Observed missing cover on ice machine exposing equipment that is collecting dust and debris. Non-food contact surfaces and equipment must be maintained in good repair. Repair or replace.
- Observed chipping/peeling paint on the side of the white standing refrigerator that contains the employee personal items, on pink shelf below bar, and peeling paper lining on chemical shelf. All non-food contacxt surfaces must be smooth and easily cleanble. Repaint or replace to allow for proper cleaning.
- Observed sanitizer solution in bucket for the bar area to be 50 ppm. Sanitizer in a bucket or a spray bottle must be at least twice the concentration of that in a 3 compartment sink. All sanitizer spray bottles or buckets must be maintained between 100-200 ppm. Employee remade sanitizer bucket during inspection.
- Observed the following floors in need of cleaning: along edges of backroom, around edges of sink room, and in the men's room. Clean as routinely is necessary.
- Observed the following areas of the floor in need of repair: damages of floor around entrance to the basement, damage to floor under ice machine, gap between wood floor and bottom of the bar. Repair to allow for proper cleaning.
- Observed the following walls/ceilings in need of repair: gap at wall juncture by the phone in the back room by beer, hole in the walk-in refrigerator wall by the thermometer. Repair to allow for proper cleaning.
- Observed visible build up of debris on the fan located in the back room by the beer cooler. Clean as routinely as necessary. Observed a dead mouse underneath the cabinet in the sink room. Conduct routine inspections underneath these types of areas to ensure all sources of contamination can cleaned as routinely as necessary.
- Observed mops stored in bucket to dry. Mops shall be handled and stored in a way that prevents contamination. By air drying mops, you allow the debris and waste water to drain away from the mop head. Air dry all mops.

- Observed damaged cutting boards in sink. Once cutting boards become damaged or discolored they must be repaired or replaced to ensure proper cleaning. Repair or replace.
