- Observed a bag of soda on the floor. The manager moved the bag of soda during the inspection. Food should be stored at least 6 inches off the ground.
- Observed cabinets throughout the front food service area to have food/debris accumulation. The surface of the chest freezer in the kitchen is dirty. Clean and maintain routine cleaning in both areas.
- Observed the hand sink near the 3 compartment sink with various utensils in it. The manager moved the items during the inspection. Hand sinks should only be used for hand washing and be easily accessible at all times.
- Observed debris build up in the kitchen and front food service area around equipment. Clean and maintain routine cleaning.
- Observed dust accumulation on ceiling vents in the front food service area. Clean vents and maintain routine cleaning.

- Paint or seal bare particle board shelves in the big storage room.
- The QUAT sanitizer spray bottle in the service line cabinet was 100 ppm. Maintain at 200 ppm. Corrected to 200 ppm.
- The insides of the ice machines, cabinets at concession stand, shelves throughout storage areas, and Eurest under counter freezer had a build up of debris. Wash, rinse and sanitize these areas.
- The outsides of the coolers and freezers in the back storage area, hand sink in the back storage area, and outsides of the cabinets had a build up ofdebris. Wash, rinse, and sanitize these areas.
- Replace the missing faucet on the right side of the 3-compartment sink.
- Found build-up of debris on floors. Clean the floors, especially under and around equipment.

- Found wrapped hot dogs in the warmer drawer at 94°F. Maintain in hot holding at 135°F or higher. Manager discarded the hot dogs and will contact a repairman to fix this unit. Do not use this unit until it has been repaired and can maintain hot foods at 135°F or higher. Call Kim at TCHD at 309-925-5511 Ext. 301 for a reinspection when repaired. Someone forgot to turn on. Corrected 135°F.
- Paint or seal the unfinished wood shelves in the dry storage area.
- The cabinets in the concession area had a build up of food, debris, and grease inside them. Thoroughly clean and sanitize them.
- The unused Delfield fridge under the Icee machine had a build up of syrup splash on the door seal and outside of it. Clean and sanitize it.
- The 3-compartment sink and adjacent hand sink were were dirty. Thoroughly clean and sanitize them.
- Found mouse feces in the lid drawer and inside the cabinet under the northwest soda dispenser at the concession stand. Clean the cabinets. Provide pest control receipts to TCHD by 6-5-15. Call Kim at TCHD at 309-925-5511 Ext. 301 for a re-inspection when completed.
