All potentially hazardous food that was kept above 41 °F for less than four hours and was cooled in an ice bath.

- Observed sandwich cooler at 60F. The top of the cooler was frozen. Unit needs to be defrosted daily due to being an older equipment.
- Soap dispenser in the Nosh section did not have soap and dispenser was not functioning because of the batteries. Ensure all soap and towel dispensers are in working order and supplied.
- Observed gnats in the large bar of the first floor. Our edge of bar has liquid accumulation from moping because bar does not have a floor drain. Liquid accumulation needs to be mopped up to eliminate the presents of gnats.
- Gnats were observed upstairs under the drink station. Liquid, dust and syrup accumulation has brought upon the gnat problem. Cleaning of this area will help remove the presents of gnats.
- Broken thermometer was inside the refrigerator in the Nosh section. Ensure all refrigeration units holding potentially hazardous foods have a working thermometer.

- Observed liquor trays inside the ice that is used for patron drinks. Ensure ice is clean and not contaminated.
- Hot water at hand sinks in the large bar area are not reaching 100 degrees F. Ensure all hand sinks reach a minimum of 100 degrees F. Remodel of bar will be completed in next 30 days.
- Hand sink stations must have soap and paper towel in bar. Ensure all hand sink stations are supplied with soap and paper towel.
- Ch 13, Art IV, section 13-112(a)(1) and (2) CFM. A food establishment shall employ at least one person assigned to each shift of twelve (12) or fewer hours who: (1) Meets the "person in charge" definition found in the Texas Food Establishment Rules; and (2) Has a valid and current food manager's certificate issued by the director. Manager must show proof of Food Manager Certification. Once a valid Food Manager's certification is obtained, proceed to 1901 S. Alamo (DBSC) Health Department desk to purchase ($15.45) a Metro Health ID.
- Observed soda gun holsters missing waste drain line. Ensure all waste drain lines are installed and connected to a sanitary sewar.
- Observed missing thermometers in refrigeration units holding milk, prepared foods, and soft cheeses. Ensure all units holding potentially hazardous foods are equipped with a working thermometer.
- Observed utensils on the dry rack with food still on them.Ensure all utensils are clean at sight and touch.
- Observed container with large burn on the bottom. Ensure all equipment is in good repair.
- Food establishment permit posted is expired. Ensure current food establishment permit is posted. A copy may be purchased for $15.45 at 1901 S. Alamo M-F 7:45-4:30
