- Ensure all hand sinks are used for only hand washing and no other purpose. Observed a beverage cup being stored inside the hand sink, located next to the french fry warmer unit, at the beginning of the inspection. Cup was moved by employees after inspection began.

- Toxic chemical (spray bottles) were hanging on condiment cart. Re-locate toxic chemicals away from food.
- Hand sink next to fries has hot water only. Work order was submitted on 07/30/2013 for repair. Any plumbing repairs must be performed by a licensed plumber and must be repaired according to law. All materials used for plumbing repairs must be of materials approved according to law. Ensure all employees are told to use rear sink for hand washing.
- Observed incorrect date marking on sliced cucumber bin in reach in cooler under hot hold unit. Observed use by first stickers but no date. Date mark refrigerated, ready-to-eat food that is frequently unwrapped or dispensed using an acceptable method.
- Observed employees switching task with out washing hands or changing gloves. Wash the hands whenever one changes from one task to another such as immediately before engaging in food preparation, working with exposed foods, clean equipment and utensils and unwrapped single service and single use articles.
- Observed scoop for ice not easily accessible (laying in ice). Ensure scoop of handle is easily accessible.

- 229.167 (e) (2) no soap.Handsoap no available at back handsink used for handwashing at time of Inspection. Handsoap was available at another sink, however sink does not have hot running water and is not approved as a handwashing sink.
- 229.165 (d) (1) (B)free of breaks, cracks. Plastic storage container in the walk-in cooler observed to be cracked. Recall all food contact surfaces must be in good condition and free from cracks and breaks.

No documentation is maintained at the food establishment that food employees contacting ready-to-eat foods with bare hands utilize two or more of the following control measures to provide additional safeguards to hazards associated with bare hand contact using incentive programs that assist or encourage food employees not to work when they are ill. Provide documentation that handling of ready-to-eat foods with bare hands is accomplished using two or more of the following safeguards: double hand washing, nail brushes, a hand sanitizer, incentive programs for sick employees not o work, or control measures approved by the regulatory authority.
- Observed employee drinks in food prep areas and not complete with straws. Place employee drinks in a designated place away from food prep areas.
- 229.165 (m) (1) (B)grease and soil accumulation. Ensure to wash, rinse and sanitize. Ensure all food contact surfaces are air dried.
- 229.163 (h) (5) handling soiled equip. Observed employees wiping down counters with sanitizing towel however food handlers are not washing their hands after handling soiled towel. Ensure to place back in proper container after use.
