- The manager properly disposed of all of the sausage links that were out of the proper temperature range. Potentially Hazardous food not at required temperature in cooling unit.
- Observed worker using bar soap encrusted with dirt and raw meat products to wash hands.
- Observed small openings and tears in the cake mix & cornmeal boxes; on the retail shelves. Also noted moderate food spillage on the shelves. Observed numerous boxes of dry goods with a use by date of 2008. Recommend to clean all food spillage, discard all damaged boxes and cans; and check all items that 2008 use by date for wholesomeness.
- Observed uncovered slice raw bacon in the walk-in cooler, lying on top of cardboard box; with condensation from the overhead compressor dripping on them. Recommend this item be disposed of properly.
- Observed, on shelves in the walk-in cooler, numerous packages, and potentially hazardous foods, packages in use, without proper date label/used by label.
- Triple sink faucet, which is also threaded, have a hose attached to it without a proper backflow preventer device installed.
- The left side of the large display reach in cooler ambient air temperature was 52°F; where as the right side of the cooler had an ambient air temperature of 36°F. Recommend that the unit be repaired at once.
- Observed improper hand washing, due to inadequate supply of handwashing soap. Recommend replacing Bar soap with liquid soap.
- Most recent graded inspection report must be posted in customer view.
- Observed that the triple sink had been improperly set up; since only one sink was in use using only clear water. Recommend to properly set up the triple sink, with active food preparation is in progress. Recommend its sanitizing pails the proper concentration of 50-100 ppm FAC If using bleach; or a concentration of 150-200 ppm, If using a Quaternary base Sanitizer.
- Person in charge did not produce a valid food manager certification card.
- Observed work wiping down the food preparation surface areas where raw fish and meat had been processed. Recommend to have a sanitizing pail, available at improper concentration whatever active food preparation is in progress. Also ensure that there are proper sanitizing test strips available. Observed discoloration, and deep scratches to all cutting boards in the meat preparation area. Recommend that all cutting boards be washed, rinsed and sanitize. Recommend to either replace or smooth plane all cutting boards with deep cuts and gashes.
