COHME is a licensed, nonprofit home health care agency, providing home care for seniors in New York City. COHME develops a specific plan of care for each client based on the client's individual needs. COHME's home health aides then implement this personalized plan of care by providing an array of social, health, and household services for the client. These may range from reading to the client, to accompanying the client to appointments or social gatherings, running errands, assisting with bathing, dressing, and eating if needed, and cooking and light housekeeping.
COHME is the only home health care organization in New York City that provides social work case management for each client at no extra cost. COHME's Licensed Clinical Social Workers and and Registered Nurses regularly evaluate all services and monitor the quality of care provided. COHME's clients and their families value the open communication they experience with COHME's home health aides, social workers and nurses, and staff. We make sure we have someone on call 24 hours a day, 7 days a week.
