    Found multiple scoops throughout the establishment to be stored with their handles touching product.
    Found the scoop to the sugar stored in the dry storage area to have a missing or broken handle.
    Please replace scoop so that there is a handle to be used for dispensing.
    Between uses during service, dispensing and incidental utensils shall be stored: in the food with handle extended out of the food, in running potable water dipper wells or clean and dry.

While doing routine inspection, the RFS 15-0518 was address. Owner stated pest control company last visited on 10/28/2015. During investigation there was no evidence of rodent activity. Case closed.

Please not that facility has more than 10 seats. Referral will be sent to county plumbing inspector.

    The nozzel of the bulk milk dispenser must be cut at a 45 degree angle to prevent the milk from holding within nozzel out of temperature.

    Found bulk milk storage dispenser with plastic outlet cut flush across. Cut outlet to milk bag at a 45 degree angle so that excess milk can drip out. •
    Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported.
  • Critical: Cross-connections, back-siphonage, back-flow prevented
    Found a hose attached to the mop sink faucet with out an antisiphonage device on it. Out let of hose was below the flood rim of the mop sink. Corrected by taking hose off faucet. Spoke to owner and asked her to place a sign above sink reminding employees to disconnect hose after use or provide an antisiphonage device on faucet.
    Devices to protect against backflow and backsiphonage shall be installed at all fixtures and equipment where an air gap of at least twice the diameter of the water inlet is not provided between the water outlet from the fixture and the fixture's flood-level rim.
  • Floors properly constructed, clean, drained
    The tiles by the ice machine are in general disrepair. FLoors shall be easily cleanable and in good repair.

This is a complaint investigation done in conjunction with a  routine inspection. RFS #13-0507 was received in regards to 'Boric acid being placed on the window sills'. During the investigation, it was noted that there was a white pasty substance outside the front door and on the non-opening sides of the windows. According to the owner, Jamy, she was utilizing 'natural solutions' as found on various websites that would assist in removing the ants. The powder mix is a concoction of powdered sugar and baking soda. In addition to this, she has had American Pest Control out to treat for and assist with destroying and mitigating areas where ants may enter and also spraying and treating for pests. The last visit from the Pest Control Operator was October 10, 2013. I did ask that continued routine calls be made to ensure that pest control operations are maintained. There were no ants present during this investigation. I did stress the importance of continued and regular pest control operations and to contact the pest control operators in between their servicing should any pests be present, as well as to review the contact to ensure that pests are covered. It is also important to note that there was a note on the front window that advised patrons that the concoction was 'baking soda and powdered sugar'.  I did inform Jamy to continue cleaning and sanitizing and to ensure that all areas are kept closed. This complaint can be closed.

    The interior of the Frigidaire freezer is soiled. The seals of the True 2 door freezer are soiled. The seals of the front freezer are soiled. The interior of the Victory milk dispenser is soiled.

Note: There is no running water dipper well, the establishment utilizes time for the storage of the mixer/blender utensils and the water is replaced every 30-60 minutes depending on use.

    The exterior of the ice machine is soiled. The shelving in the ice machine room is soiled.
