Several flies (8-10) were observed throughout the restaurant. Maintain pest control measures
  • Critical: Cold food at proper temperatures during storage, display, service, transport, and cold holding
    The walk-in freezer was found with a thermometer temperature of 50 degrees F. Yogurt was found stored at a temperature of 49 degrees F in walk-in cooler. Several other food products were found at a temperature greater than 41 degrees F (see voluntary destruction form).
    Temperatures of all potentially hazardous foods shall be at 41°F or below, or 135°F or above, at all times except as provided for in the Code.
    Corrected by taking the temperature of all food products in the walk-in cooler with the manager on duty. The sanitarian calibrated 2 thermometers at 10:55 a.m. before beginning to take temperatures. All foods found above 41 degrees were discarded by the manager. The manager also took the temperature of several food products to confirm the products were above 41 degrees. Maintenance was called in to remove significant amounts of ice build-up behind the blowers in the walk-in cooler. See voluntary destruction form.. The cooler has been repaired and the temperature has dropped to 40 degrees during the time of inspection. Corrected

    Bathroom cleaner and other cleaners on the metal racks across from the mop sink was found stored above ready-to-eat packets of ketchup and next to other food products on the same shelf
    Poisonous or toxic materials necessary for the maintenance of the establishment shall not be stored above or intermingled with food, food equipment, utensils, or single-service articles.

Spoke to Manager Mila in regards to complaint. Manager stated they have had previous issues with pest management. Manager has been in contact with Pest Control Services, Markley Pest Control. Manager stated pest control was on premisis on 9/16/2015 setting bait stations. Please clean floors around all fixed equiptment ( Co2 machine, used oil storage tanks, and plumbing fixtures by back door). Inspector noticed cooler unit on service line leaking water onto floor. Inspector noticed all seals around doors to be in good repair. Inspector did not find any openings into the building when inspecting outside premisis. Inspector did not observe any evidence of rodents or rodent activity. Building is located next to open fields in areas of recent construction. Clean and detail soda machine area and cabinents. Thank you for your cooperation.

9/17/2015Complaint Investigation
A 1st recheck was conducted on 08/13/15 to determine if the quantity of flies from previous inspection/complaint has decreased and if the air curtain in 1st drive through window was replaced or fixed. It was found during this inspection that the number of flies present in the kitchen and dining area have decreased. The air curtain was observed to be working well during time of inspection. The manager states pest control came out on 08/11/15 to treat for a heavy amount of crickets outside. There was no treatment for flies recorded on invoice during time of pest control visit due to the decreased amount of flies. The decreased amount of flies is likely attributed to the fixed air curtain and new glue strips helping reduce numbers.

Continue to reduce the number of flies by maintaining the air curtain in good repair and replacing glue strips frequently. Cover all open containers of food when not in use to prevent insect entry.

A recheck fee will be assessed.

Case closed. No further investigation needed.

8/13/20151st Recheck
This complaint is in regard to flies, bare hand contact with ready-to-eat food, and uncovered food. At the time of the investigation, all employees were observed to be wearing gloves and no bare hand contact with food was seen. Multiple flies (around 20) were seen in establishment. In addition, fly strips have been hung (around 4 strips) and flies were observed on the strips. Food products normally uncovered for quick sandwhich making were observed to still be uncovered despite multiple flies in and around open containers. During investigation air curtains were found to be turned off with open drive through windows.

Corrective actions: cover all open containers until the number of flies is decreased and multiple flies are no longer visible
  • Critical: Presence of insects/rodents. Animals prohibited
    Observed multiple flies in facility
    Contact pest control provider specifically for fly treatment within 10 days
    A recheck will be conducted within 10 days (on or after August 1st, 2015) - a recheck fee will apply

  • Food contact surfaces designed, constructed, maintained, installed, located.
    Found plastic food contact surfaces in disrepair: plastic lid melted, cracks in several plastic pans, stickers left on clean plastic pans. Food contact surfaces shall be in good repair.
  • Non-food contact surfaces clean
    Non food contact surface cleaning: lime accumulation on the side of the soda dispenser in the dining area, clean build up off the counter behind the soda dispenser in dining area, found a pile of ketchup on the roll out self in the cabinet under the ketchup dispeser, clean the outside of the McFlurry candy dispensers, clean as needed.
  • Plumbing installed and maintained
    Found hot water to mop sink not turning off all the way. Might be a bad gasket in the faucet. Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
  • Critical: Hand washing sinks installed, located, accessible
    Found bulk tea container stored in front of handsink by three compartment sink. Corrected by speaking to employees and moving bulk tea container.
    Lavatories shall be accessible to employees at all times.
  • Containers covered, adequate number, insect and rodent proof, emptied at proper intervals, clean
    Dumpster is missing left side lid. Observed several bird around dumpster area. Waste containers stored outside the establishment such as dumpsters, compactor and compactor systems shall be kept covered when not in actual use.

    Checked two sani buckets holding towels with no detectable level of quat or bleach. Informed manager who had someone change sani buckets.
    Kitchenware and food-contact surfaces of equipment shall be washed, rinsed, and sanitized after each use and following any interruption of operations during which time contamination may have occurred.
  • Wiping cloths clean, used properly, stored
    Several wet wiping cloths seen on counters throughout the facility.
    Moist cloths or sponges used for wiping food spills on kitchenware and food-contact surfaces shall be stored in a sanitizing solution between uses.

    Area by the coffee preparation stands and the mcflurry machine is soiled with coffee/ice cream/food debris.

|Complaint # 14-0111 was addressed with the manager on duty, Amanda. We discussed the fact the gloves can give employees a false sense of security and that they should be treated as a utensil. SHe did say that they will address this at the next staff training meeting.

3/31/2014Complaint Investigation
On l01-15-2013 complaint # 14-0024 was addressed with the General Manager, Teresa. Hair restraint was discussed and upon observation it was noted that all employees had proper hair restraints in place.
Handwashing was addressed and the proper procedure was reviewed, discussed and  HACCP flyer was left for the facility to use for training, copying etc. Hand washing not only before donning gloves but AFTER glove removal was stressed.
No violation noted during this evaluation.
1/15/2014Complaint Investigation
Sanitarian responding to emergency call of a carbon dioxide leak at the establishment. Administration Greg Chance received an after hours call from the Peoria Fire Dept about a carbon dioxide leak reported by McDonald's manager Theresa Gramajo. Peoria Hazmat responded to the call immediately at 5:00 am. After speaking with Ms. Gramajo, it is reported that the carbon dioxide alarm that monitors the industrial soda machine detected a leak on the machine. A gasket had become loose. Peoria Fire Dept and Hazmat team arrived on site to assess the safety of the of the establishment. Carbon dioxide levels were not found to be hazardous. Establishment was able to continue normal operations. Upon inspection, soda machine was not in use, and a Coke-Cola serviceman was in the process of serving the machine and resetting the alarm. Establishment is using bottle soda until soda machine is safe to use. No food, single-service items, or food contact surfaces were compromised during the present of the carbon dioxide leak.
No violation noted during this evaluation.
