KDA_3_30212 - --Located on the pantry shelving for other food items, there was one glass mason jar with yellow substance and no label. PIC said he thought it was honey but he did not know where it came from since it was in a glass canning style jar. COS. Substance was dumped out. KDA_3_50118A1 - --Located in the RIC, there were several PHF food items being held more than 7 days: house made slaw 11/5 hummus 11/5 pulled pork 11/4 duck gravy 11/5 corn ju 11/11 COS. All items were dumped in the trash. KDA_3_50211B - --Located in the chest freezer, there were 25 pounds of cured pork that had been cured with nitrates added and the FE does not have a HACCP plan or variance. COS. Pork was placed in dumpster. KDA_4_20211A2 - --Located on the racks for clean dishes, there were 2 wooden spatulas and 1 rubber spatula with cracks, splinters, and melted areas. COS. Items were placed in trash can. KDA_4_60111A - --The potato fry cutter has an accumulation of black dried food debris on the food contact surface. PIC did not know when it was last used or cleaned. NOTE: this is a repeat violation, however it was noted under violation 4-601.11(C) on last inspection KDA_4_60211E4 - --The ice machine guard plate has visible black/purple mold on the edges. KDA_5_50117 - --The ladies toilet room does not have covers on the trash cans. KDA_6_30311A - --There are several lights not working in the dish area where the 3 vat sink is located and where clean dishes are stored. The area is very dim and dark and it is not adequate lighting to see without using a flashlight. KDA_6_501112 - --Located in the room where the WIC for beer is located, there were 6 dead roaches on the floor. KDA_6_501114A - --There is clutter and unused equipment in the exit hallway of the FE. There is clutter, trash, and old equipment located in the storage area on the second level. NOTE: During the last inspection, the owner was asked if there was any other storage areas such as a basement or attic or upstairs that would need to be looked at. The owner said there was not. During the inspection today, the employee stated that she was also here on November 15, and the owner purposely locked the door and told them not to say anything about the storage room upstairs. KDA_7_10211 - --Located on the shelving with other chemicals in the kitchen, there was one spray bottle of clear liquid with no label. PIC was not sure what it was. COS. PIC was able to give dates on items. pulled pork November 1, house salsa Nov 1, potatoes Nov 4. COS. All items were dated. KDA_3_50211B - --Located in the RIC and RIF there were pork items, such as 10 pounds bacon and 5 pounds pork belly cubes, that had been cured with nitrates added. PIC said they do this process on site in the FE. FE does not have a HACCP plan or variance. COS. Educated staff on getting a variance and HACCP plan. These foods were removed from the FE by the owner and taken home. KDA_4_20211A2 - --Located on the shelving for clean dishes there were 3 out of 100 china plates that had chips on the edges. COS. Plates were placed in trash can Located on the rack for clean utensils, there were 2 rubber spatulas that had chips and cracks on the edges. COS. Items were placed in trash can. KDA_4_60111C - --Located at the soda nozzles, the rubber nozzle holders have visible pink mold in the bottom. The ice machine exterior and lid has an accumulation of sticky debris. The cutting boards on the make tables have scratches and black debris. The fry cutter has an accumulation of dried food debris. Employee said it was last cleaned on November 1. Located in the bar area, the RIC for kegs have an accumulation of sticky debris on the bottom shelving. The 3 floor fans have an accumulation of lent debris. KDA_4_60211E4 - --The ice machine guard plate has visible black mold on the edges. KDA_5_20515B - --The hand sink in the cooking area is very loose and falling off the wall. PIC has a repair person called. KDA_5_50117 - --The ladies toilet rooms do not have covers on the trash cans. KDA_6_30311A - --There area several lights not working above the 3vat sink area and it I every dim and dark. There is also a RIC and shelving for clean dishes in this area. KDA_6_50111 - --There are several places in the kitchen and storage areas that have peeling paint off the ceilings. KDA_6_501112 - --Located behind the bar area there was one dead mouse laying on the floor. Located in the room where the WIC for beer is located, there were 3 dead roaches. KDA_6_501114A - --There is clutter and unused equipment in the exit hallway of the FE. KDA_6_50112A - --The floor in the bar area has an accumulation of black sticy debris. The back door has an accumulation of black debris. The door to the hot water tank has an accumulation of dirt debris. The floor area along the floor molding and under the ice machine have an accumulation of black debris. KDA_7_10211 - --Located in the kitchen area there was one spray bottle of clue liquid with no label. PIC said it was window cleaner. COS. Bottle was labeled.
