- KDA_3_30412B - --In the storage room, the sugar containers scoop handle was touching the sugar. COS-scoop was removed.
- KDA_Defacto - --GE reach in, shredded cheese 39.4F. Two door reach in, cooked on site rice, 43F. They had the rice out for about 10 minutes at 10:00AM today.

- KDA_2_30114H - --Employee was handling money and then put on gloves to make food with out washing hands. Educated on proper hand washing.COS-Gloves were discarded, hands washed and new gloves put on.
- KDA_3_30211A1a - --In the two door cooler, there were two pans of raw chicken that did not have a separation between them and the bottles of pop and the containers of made on site salsa. COS-Salsa and pop were moved to another shelf. --In the two door reach in, there was raw hamburger sitting directly on top of made on site single serve red sauce. There was raw shell eggs sitting directly on top of whole tomatoes. In the single door cooling unit, raw steak in a thank you bag was sitting directly on top of shredded cheese. COS-Eggs, steak and hamburger were moved to the bottom shelf.
- KDA_3_30212 - --In the storage room, there was a container with a white substance in it. Person in charge said it was sugar. There was also a spray bottle with a red liquid in it. Person in charge said it was red dye. COS-containers were labeled.
- KDA_3_30412B - --In the storage room, there was a container of sugar that had a scoop with a handle in it. The handle was touching the sugar. Educated on scoops not touching food product. COS-handle was placed above the sugar.
- KDA_3_50116A2 - --Sitting on the prep table, tamale's 58.5F. Employee said they had been out on the prep table since 8:00AM. They were getting ready to put them in Ziploc bags. Tamale sauce was sitting on the prep table at 56.9F. Employee said it had been out for about an hour and half. COS-tamales were bagged and put in coolers. Sauce was put on stove top and heated to 165F.
- KDA_3_50118A1 - --In the white reach in, there was a plate of Chiles Rellenos dated with a prepared date of 10-8-15. Product was held over seven days. COS-Rellenos were discarded.
- KDA_3_50118A2 - --In the white reach in, shredded lettuce, ground beef with potatoes and peas and spaghetti were with out a date. Employee said these were prepared two days ago. In the two door reach in, red Mexican soup was without a date. Person in charge said it was on Sunday. COS-Lettuce and spaghetti was discarded, ground beef and soup was correctly dated.
- KDA_4_20211A2 - --In the two door reach in cooler, there were cracked lids on mole sauce and refried beans. Sitting under the prep table, there were three plastic containers with cracks along the food contact surface. COS-lids on the food were replaced. Containers were discarded.
- KDA_4_60111A - --The microwave by the single door reach in had an accumulation of dried on food debris.
- KDA_5_20511A - --In the hand washing sink, there was a small pot sitting in the basin. Inspector had to move the pot before hands could be washed. COS-Pot was moved to three compartment sink.
- KDA_5_50117 - --In the unisex restroom, there is not a covered trash can.
- KDA_6_50112A - --The wall behind the prep table, has dried on food debris.
- KDA_7_10211 - --In the mop sink, there was a spray bottle with a green liquid in it with out a common name. Person in charge said it was degreaser. COS-bottle was labeled.
- KDA_Defacto - --Frozen food was frozen. --In the three compartment warmer, ground beef 184.6F. --Reheating in a three compartment warmer: made on site rice 147.4F.
