  • KDA_6_20215A3 - --There a 1/4 inch gap under the back emergency exit door in the receiving area large enough to allow the entry of pests. No activity.
  • KDA_Defacto - --There is a hose attached to the mop sink without additional backflow prevention downstream from the atmospheric vacuum breaker and the hose was lying below the flood rim of the mop sink. The AVB was not leaking. Recommended installing a wall hanger so that the hose hangs above the flood rim level.
  • KDA_KSA65_689a - --The facility has been operating since 8/13/15 without a valid license. The new food establishment application and credit card payment form were faxed/emailed in today 11/4.
