A friend of mine retired and moved to an European country. Having light form of dementia, he did not care to do research and find out out that he was supposed to submit form SSA-21 within 30 days of the move. Now being there for more than a year, he asked what is the best way to handle it. Should he submit the form and indicate that he moved a month ago or submit the form with the accurate move date. In the former case any way somebody will try to validate and in the latter would he be penalized?
Your friend should contact the nearest US Embassy to speak with their Federal Benefits Unit (FBU). FBU's are there to assist US citizens with such issues.

Since your friend will have to sign the form with this statement, I wouldn't recommend lying on the form.
"Under penalties of perjury, I declare that I have examined the information on this form and to the best of my knowledge and belief it is true, correct, and complete. I understand that anyone who knowingly gives a false or misleading statement about a material fact in this information, or causes someone else to do so, commits a crime and may be sent to prison, or may face other penalties, or both."

we may stop your benefits. For the 1st violation, your
benefits will stop for 6 months; for the 2nd violation, 12

So sounds like he can expect up to 6 months of lost benefits
