All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Fryer door damaged. Must repair or replace fryer door.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Must replace unclean ice machine plastic tube(upper portion of ice machine).
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Must detail clean floor throughout shed.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Must replace damaged ceiling light covers (X2) above 2nd floor prep area.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Employees washroom handwash bowl pipe draining slowly. Must repair and maintain sink.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Basement storage room cluttered. Must elevate all items 6' off floor or remove items to prevent potential pest harborage.

All cold food shall be stored at a temperature of 40F or less. OBSERVED POTENTIALLY HAZARDOUS FOODS AT IMPROPER TEMPERATURE. OBSERVED CHEESE FONDUE STORED IN THE 1ST FLOOR PREP COOLER AT 53F, AND MASHED POTATOES STORED IN THE SAME COOLER AT 46F. MANAGEMENT VOLUNTARILY DISCARDED 10# OF FOOD WORTH $13. CRITICAL VIOLATION 7-38-005A.
Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. OBSERVED THE HIGH TEMPERATURE DISHMACHINE IN THE 1ST FLOOR DISHWASH AREA IMPROPERLY MAINTAINED. FINAL RINSE TEMPERATURE WAS AT 155-160F. DISHMACHINE WAS NOT BEING USED AT THE TIME OF THE INSPECTION. ECOLAB SERVICE TECH ONSITE DURING THE INSPECTION TO REPAIR THE MACHINE. HE STATED THAT IT COULD NOT BE FIXED AT THIS POINT. A NEW BOOSTER HEATER WILL NEED TO BE INSTALLED. HE WOULD HOOK UP THE MACHINE TO A CHEMICAL SANITIZER UNTIL THE BOOSTER IS INSTALLED. MACHINE TAGGED HELD FOR INSPECTION. NO CITATION ISSUED. CRITICAL VIOLATION 7-38-030.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES BEHIND THE BAR AND IN BOTH THE 1ST AND 2ND FLOOR PREP AREAS. APPROXIMATELY 50 LIVE FRUIT FLIES WERE OBSERVED IN THESE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

REPLACE BROKEN WALL TILES IN THE 1ST FLOOR DISHWASH AREA. DETAIL CLEAN THE WALLS AROUND THE COOKING EQUIPMENT IN THE 2ND FLOOR PREP AREA.

A separate and distinct offense shall be deemed to have been committed for each Serious violation that is not corrected upon re-inspection by the health authority. PREVIOUS SERIOUS VIOLATION FROM REPORT #612467 ON 9/21/11 NOT CORRECTED: 18/01- OBSERVED LIVE FRUIT FLIES BEHIND THE BAR AND IN BOTH THE 1ST AND 2ND FLOOR PREP AREAS. APPROXIMATELY 15 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS AND CEILING IN THESE AREAS. ECOLAB WAS OUT FOR A PEST INSPECTION ON 9/27/11. ECOLAB TECHNICIAN TO ARRIVE ONSITE WITH FINITO SPRAY TO KILL ADULT FLIES. MANAGER ALSO USING VACUUM FOR ANY VISIBLE FLIES ON THE WALLS. SUPERVISOR LOPEZ NOTIFIED OF CRITICAL VIOLATION. CRITICAL VIOLATION 7-42-090.

EXTERIOR OF ALL GARBAGE BINS NEED DETAILED CLEANING; PIPES UNDER MOST SINKS MUST BE KEPT CLEAN ALSO.
FLOOR DRAINS THRU-OUT PREMISES MUST BE FREE OF FOOD DEBRIS/DIRT. FLOORS AROUND CORNERS UNDER EQUIPMENT, MUST BE KEPT CLEAN ALSO.
MISSING/BROKEN BASEBOARDS TILES IN DISHROOM & PREP LINE MUST BE REPLACED. DUSTY CEILING VENTS IN DISHROOM MUST BE CLEANED & MAINTAINED.
A HOT & COLD WATER LINE MUST BE INSTALLED BY THE ICE CREAM MACHINE. (UNIT TAGGED)
