The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO REMOVE PEELING PAINT AND REPAINT WALLS AND CEILING IN DISH ROOM AREA.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. INSTRUCTED TO PROVIDE LIGHT SHIELD WITH END CAPS ABOVE FOOD PREP AREA/KITCHEN AND IN DISH WASHING AREA.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO REMOVE ALL UNNECESSARY ARTICLES STORED IN THE BASEMENT. ALL USABLE ITEMS MUST BE STORED 6' OFF THE FLOOR FOR EASY ACCESS ON CLEANING.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN THE FOLLOWING: INTERIOR AND EXTERIOR SURFACES OF DISH MACHINE, ICE MACHINE, KITCHEN COUNTER TOPS, PREP AND STORAGE SHELVES.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. INSTRUCTED TO PROVIDE TIGHT FITTING LID FOR ICE BIN BEHIND BAR.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN FLOORS ALONG THE WALLS AND IN ALL CORNERS IN THE KITCHEN AND IN BASEMENT.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. NOTED ICE SCOOP STORED ON TOP OF ICE MACHINE. INSTRUCTED TO PROPERLY STORE IN A CLEAN SEPARATE CONTAINER TO PREVENT CONTAMINATION.

Interior of ice machine not cleaned or maintained. Observed black mold substance inside ice machine interior panel and parts. Instructed that ice machine must be detailed cleaned and sanitized. 'Held for Inspection' tag posted on ice machine. Serious citation issued 7-38-005(A).
Previous minor violation s not corrected 10-13-2010, report #419441: -32 Instructed to provided tight fitting lid for ice bin behind bar. -33 Instructed to clean and maintain the following:interior and exterior surfaces of dish machine, ice machine, kitchen counter, prep and storage shelves. -34 Instructed to clean floors along the walls and in all corners in the kitchen and in basement. -35 Instructed to remove peeling paint and repaint walls and ceiling in dish room area. -36 Instructed to provide light shield with end caps above food prep area/kitchen and in dish washing area. -41 Instructed to remove all unnecessary articles stored in basement. All unused items must be stored 6 inches off floor for easy access on cleaning. -43 Ice scoop stored on top of ice machine. Instructed to properly store in a clean separate container to prevent contamination. Serious citation issued 7-42-090.
Drainline not provided at soda dispenser in bar area. Instructed to install waste drainline. Ice machine condensation line draining to one compartment sink. Instructed to remove drainline from draining inside sink.
Can opener attaached to prep table, cooking equipment, soda gun holders not cleaned. Instructed to detail clean and sanitize.
Walls in toilet rooms and around exterior door frames not maintained. Instructed to repair walls and reseal around door frame and replace missing wall tile in toilet rooms.
Pest control logbook not provided or maintained on premises. Outer openings not protected or rodent proofed. Side exit doors has 1/4 inch gap in corners of doors and pest control logbook not avaialble during inspection. Instructed that all three exterior must be repaired to prevent rodent and insect entry and must have logbook available during inspection. Serious citation issued 7-38-020.

Unnecassary articles stored in walk-in coolers for dry storage in basement not maintained. Instructed to remove all unsed articles and elevate supplies 6 inches or higher and away from the walls.
