The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN FLOORS THROUGHOUT, UNDER/BEHIND ALL HEAVY EQUIPTMENT, IN CORNERS REMOVING ALL DEBRIS & HEAVY BUILD-UP IN CORNERS AND ALONG WALLS.

OBSERVED 15-20 MICE DROPPINGS SCATTERED IN FRONT PREP AREA ALONG FRONT LEDGE, INSIDE FILE CABINET, ON WOODEN PLANK UNDER STEAM TABLE AND FLOOR ALONG WALLS IN NORTHEAST CORNER. INSTRUCTED MANAGER TO CALL AN EXTERMINATOR FOR SERVICE, CLEAN AND SANITIZE ALL AREAS. SERIOUS VIOLATION 7-38-020 CITATION ISSUED.
