No certified food manager know premises when potentially hazardous foods are being prepared and served. Establishment does not have a certified manager or city of Chgo cert. Instructed manager that a certified food manager must be on premises during all hours of operation and have have certificate from city of Chicago.
Floor along wallbase in kitchen, storage and boiler room not cleaned. Instructed to detail clean.
Ventilation not provided in several toilet, shower room and toilet on 1st floor has board over window. Instructed to repair exhaust fans in toilet rooms and remove board from window to have window open and close.
Coving along wallbase in hallway missing in areas. Instructed to replace missing rubber coving. Faucet in wall in utility closet on third floor not sealed. Instructed to seal holes in walls. Plaster on walls crakced in utility closet, hallway and kitchen storage room. Instructed to smooth surface and repaint. Light covers in kitchen not cleaned. Instructed to detsil clean.
