All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE TABLETOP ABOVE THE 1ST FLR KITCHEN IS UNSANITARY AND NEEDS TO BE REPAired so it is easy to clean.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Rout the dipping well drain to a sanitary drain. Install drain lines in all soda gun holders.

All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. The salad preparation refrigerator is not working and iced cheeses, cooked vegetables and dairy based dressings holding 48-49 f temperatures were discarded at this time and the salad cooler was tagged held for inspection until repaired or replaced and inspected by the Chicago Department of Public Health. Fax a tag removal request to (312) 746-4240 when the equipment is ready for re-inspection. -OR- E-mail a tag removal request to cicero_marc@cdph.org and salazar_jairo@cdph.org when ready for re-inspection.

MUST REPAIR LEAKY FAUCETBASE AND FAUCET HANDLES AT 3COMP SINK IN THE FRONT SERVICE BAR AREA AS WELL AS AT THE 3COMP IN THE BASEMENT. MUST ALSO CLEAN AND REMOVE GRIMMY BUILD UP AROUND EXPOSED HANDSINK FAUCET HANDLES NEXT TO WOODBURNING GRILL. MUST CLEAN EXHAUST HOOD VENT FILTERS.
MUST REPAIR BROKEN FLOOR TILES AROUND THE GREASETRAP BOX UNDER 3COMP SINK IN THE BASEMENT.ALSO MUST REGROUT FLOOR AT DISHWASHING MACHINE AREA.
MUST SEAL HOLE AROUND PIPE AT WALL UNDER DISHWASHING MACHINE AREA.
