OBSERVED TWO OPEN BASKETS OF CUT READY TO EAT BREAD, OPEN BOWL OF SAUCE AND OLIVE OIL, UNCOVERED, UNPROTECTED FROM CONTAMINATION FROM PUBLIC. INSTRUCTED ALL FOODS ON DISPLAY FOR SAMPLES MUST BE COVERED, PROTECTED FROM THE PUBLIC. ALL FOOD DISCARDED.
All dish washing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. OBSERVED PRODUCE PREP TWO COMPARTMENT AND EXPOSED HAND SINK WITH NO HOT WATER. TEMPERATURE FOUND AT 86.7F AND 87.9F. INSTRUCTED TO USE MEAT PREP EXPOSED HAND WASHING SINK AND DELI PREP SINK FOR ALL USES UNTIL HOT WATER OBTAINED/PLUMBING REPAIRED.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DAMAGED AND MISSING FLOOR TILES UNDER DELI PREP OVEN, MUST REPLACE. FLOORS ALONG BASEBOARDS THROUGHOUT DELI AND MEAT PREP WITH EXCESSIVE FOOD/DIRT DEBRIS, MUST CLEAN. MUST PROVIDE FLOOR DRAIN COVER IN PRODUCE AT WEST WALL OF PRODUCE AREA. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. WOODEN BUTCHER BLOCK PREP TABLE IN MEAT CUTTING ROOM WITH DEEP BLACKENED CUTS, IN POOR REPAIR. MUST REPLACE.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. KNIVES FOUND IMPROPERLY STORED AT DELI HOT HOLDING PREP CUTTING BOARD. MUST PROVIDE PROPER KNIFE HOLDER.

Adequate and convenient hand washing facilities shall be provided for all employees. NO PAPER TOWELS IN DISPENSER AT DELI PREP EXPOSED HAND WASHING SINK. EMPLOYEE PROVIDED DURING INSPECTION.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MANAGEMENT INSTRUCTED THAT HOT WATER ABOVE 110F IS NEEDED TO WASH DISHES AND UTENSILS. MANAGER CALLED A PLUMBER DURING THE INSPECTION TO CHECK THE HOT WATER HEATER. BY THE END OF THE INSPECTION, THE WATER TEMPERATURE WAS OVER 110F AT THE 3 COMPARTMENT SINK. CRITICAL VIOLATION 7-38-030.
FOOD NOT PROTECTED DURING PREPARATION. OBSERVED FRESH SQUEEZED JUICES (ORANGE AND GRAPEFRUIT) FOR SALE THAT IS SQUEEZED AND BOTTLED ON THE PREMISES. THE BOTTLES ARE MANUALLY CAPPED. MANAGEMENT INSTRUCTED TO REMOVE JUICES FOR SALE AND JUICER WAS TAGGED HELD FOR INSPECTION. SERIOUS VIOLATION 7-38-005A.
OBSERVED LIVE FRUIT FLIES IN THE DELI PREP AREA, PRODUCE PREP AREA AND PRODUCE AISLE. 15 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS, CEILING TILES AND COLUMNS IN THESE AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
