A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. Previous minors from 6/8/09 visit, not corrected at this time: #32-cutting boards with deep/dark grooves must be replaced, #34-must elevate all stock off the floor at the baar area, #36-must provide light shields or shatter-proof bulbs above sinks & prep table at the bar, #38-must fix leaky pipe under middle sink of 3-comp in main kitchen.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Following need cleaning: cabinets under sinks in main kitchen, interior panel of ice machine, liners on shelves at main dishroom- due to food stains/debris/dirt/dust.
The walls and ceilings shall be in good repair and easily cleaned. Missing ceiling tiles in mens locker /bathroom must be replaced; filters at hood of cooking equipment must be free of dust/grease build-up.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Must fix a leaky pipe at coffee station sink & address condensation build-up on 3-coolers (bar & 38th floor).

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. PREP AREA REACH IN DOOR COOLERS AND WALK IN COOLERS DOOR GASKETS RIPPED/MISSING. MUST REPAIR AND MAINTAIN COOLER DOOR GASKETS.

OBSERVED STANDING WATER ON FLOOR(BY FLOOR DRAIN) IN DISHMACHINE ROOM AND FOOD DEBRIS ON FLOOR IN PREP AREA UNDER AND AROUND COOKING EQUIPMENT. MUST DETAIL CLEAN FLOOR/KEEP FLOOR DRY THROUGOUT PREP AND DISHMACHINE AREA.
