Found fruit fly activity at pastry kitchen by walk in cooler.(observed about 30 fruit flies on walls and flying around. serious violation. citation issued.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not swept at main kitchen areas/pastry kitchen.
Found no backflow prevention devices at 2 ice machines and coffee machines on 3rd/pastry kitchen.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 3RD FLOOR PREP AREA HANDWASH BOWL DRAINNG SLOWLY(ALONG EAST WALL) AND 1-COMPARTMENT SINK FAUCET LEAKING AT BASE(AT SAME LOCATION). MUST REPAIR AND MAINTAIN SINKS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE THERMOMETERS INSIDE PREP AREAS REACH IN COOLERS.
