OBSERVED LIVE FRUIT FLIES IN THE THE BAR AREA. APPROXIMATELY 15 LIVE FRUIT FLIES WERE OBSERVED ON THE LOWER WALLS,AND ON THE EQUIPMENT IN THIS AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. ALSO, THE 1ST FLOOR SOUTH DOUBLE DOOR AT BOTTOM CENTER, DOORS AT THE 2ND FLOOR PARTY ROOM ARE NOT RODENT PROOFED. THERE IS A 1/4' GAP MODE OF ENTRY FOR PESTS.ALSO PEST CONTROL LOG BOOK MUST BE PROVIDED WITH A MAP LOCATOR TO IDENTIFY PEST CONTROL SERVICES.SERIOUS VIOLATION 7-38-020.
Cutting boards with deep, dark grooves must be sanded/bleached or replaced and repair/replace interiors of washbowls at the customers toilet rooms.
THE FOLLOWING REQUIRE A DETAIL CLEANING SURFACES/SHELVINGS ABOVE THE COOKLINES,INTERIORS OF BAR COOLERS,BAR GUNS AND HOUSINGS SAME AREA,UPPER PANEL OF ICE MACHINE,EXTERIOR SURFACES OF THE DISHMACHINE.
REPLACE BROKEN FLOOR TILES IN KITCHEN AREAS WERE NOTED AND FLOOR IN BAR AREA REQUIRE DETAIL CLEANING.
LIGHT SHIELDS IN THE KITCHEN AREAS REQUIRE A DETAIL CLEANING.
THE HOOD/FILTERS OVER THE COOKING EQUIPMENTS,THE VENTILATION COVERS THROUGHOUT THE PREMISES REQUIRE CLEANING AND REPAIR THE MECHANICAL VENTILATION INSIDE THE TOILET ROOMS A BACKFLOW PREVENTION DEVICE IS NEEDED AT THE HOT WATER DISPENSER AT REAR SERVICE TABLE.
