Soft serve Taylor machine not maintained or operated. Hot and cold water not provided for top of machine. 'Held for Inspection' tag placed on machine. Notice must be faxed to Chgo Dept. of Public Health when machine has running hot and cold water. Soft serve Taylor machine can not be used until 'Held for Inspection' tag must been removed.

Walk in cooler shelving that had exposed raw wood, shall be repair, sealed.
