Adequate and convenient hand washing facilities shall be provided for all employees. MUST PROVIDE AND MAINTAIN SOAP AND HANDTOWELS AT EXPOSED SINKS BEHIND BOTH BARS.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED TWENTY RODENT (MOUSE DROPPINGS) IN REAR DRY STORAGE AREA BY THE KITCHEN, MUST REMOVE. SEAL HOLE IN FLOOR IN CORNER OF ROOM WHERE DROPPINGS OBSERVED.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CITY OF CHICAGO FOOD SERVICE MANAGER ON DUTY AT TIME OF INSPECTION. KITCHEN WAS OPEN AND OPERATING. INSTRUCTED THAT A CITY OF CHICAGO FOOD SERVICE MANAGER MUST BE ON PREMISES AT ALL TIMES FOOD IS BEING PREPARED AND SERVED.
