No certified food manager on premises or certificate posted when potenially hazardous foods are served-beans at 150.2 F. Instructed that a certified food manager must be on premises at all times and have a City of Chicago certified manager certificate posted. Seriuos citation issued 7-38-012.
No pest control log book provided or maintained on premises. Establishment has Terminix Pest COntrol, however company only provided service invoice. Instructed that a pest control log book must be maintained for establishment and have all required documents. Serious citation issued 7-38-020.
Food handlers and employees in prep area not wearing hair restraints. Instructed that all employees in prep area must have on proper hair restraints.
Light not working under exhaust hood. Instructed to replace non-working bulb.
Thermometer not working in basement walk-in cooler. Instructed to provided thermometer conspicuous inside cooler.
