All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST PROVIDE STOPPERS AT 3-COMP SINK. MUST REPLACE BROKEN GLASS DOOR OF A REACH-IN FREEZER AT REAR PREP AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. SLOW-DRAINS AT FRONT HANDSINK & AT REAR PREP SINK MUST BE FIXED.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE INTERNAL THERMOMETERS IN MOST COOLERS.

SHELF SURFACE PEELING (ABOVE 3-COMPARTMENT SINK) AND PREP AREA REACH IN COOLER DOOR RUBBER GASKET RIPPED. MUST REPLACE DAMAGED SHELF SURFACE AND REPAIR/REPLACE DAMAGED COOLER DOOR GASKET.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR TILES DAMAGED/CRACKED AT PREP AREA ALSO OBSERVED STANDING WATER ON WASHROOM FLOOR. MUST REPAIR DAMAGED FLOOR TILES IN PREP AREA AND KEEP FLOOR DRY IN WASHROOM.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. PLASTIC FLOOR BASE PEELING FROM WALL UNDER AND AROUND 3-COMPARTMENT SINK. MUST REATTACH TO WALL/SEAL ALL GAPS BETWEEN COVER AND WALLS.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED FOOD AND WATER STAINED CEILING TILES ABOVE PREP AREA. INSTRUCTED MANAGER TO REPLACE STAINED CEILING TILES.

FOLLOWING NEED CLEANING: HEAVY ICE BUILD-UP ON CHEST FREEZER IN REAR, DUSTY FAN GUARDS OF COOLERS, & SINK DRAINS AT BOTH HANDSINK & 3-COMP SINK (IN REAR)-DUE TO FOOD DEBRIS.
THE REAR HANDSINK IS SLOW-DRAINING, MUST FIX.
ONE (OF TWO) SLIDING GLASS DOORS OF REAR CHEST FREEZER IN MISSING (CARDBOARD COVER BEING USED), MUST REPLACE. CUTTING BOARDS WITH DEEP, DARK GROOVES MUST BE REPLACED ALSO.
