A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. PREVIOUS MINOR VIOLATION FROM JAN.7/09 (REPORT#156081) #38-REPAIR LEAKING 3-COMP SINK STOPPER LINE/GASKET & INSULATE DRAIN LINES ABOVE 3-COMP SINK/PREP TABLE/CLEAN POTS & PANS STORAGE RACK, OR INSTALL A DROP-CEILING TO PROTECT PREPARATION & DISHWASHING AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS ALONG WALLBASES AT NORTH WALL OF 2ND-FLOOR PREP/DISHWASHING AREA; UNDER STORAGE SHELVES AT 1ST-FLOOR, MUST BE FREE OF FOOD DEBRIS. ALL STOCK/SUPLLIES MUST BE ELEVATED OFF THE FLOOR AT REAR HALLWAY & AT ANY STORAGE AREA.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ANY SODA/MILK CRATES USED FOR STORAGE RACKS MUST BE REMOVED & REPLACED WITH REGULAR SHELVES, FOR EASY ACCESS TO CLEANING/PEST MONOTORING.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. NOTED LEAK UNDER THE SODA DISPENSER CABINET-MUST FIX.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST RE-SEAL CRACKS/GAPS AT SERVING COUNTER OR ANY LOOSE BASEBOARD TILES.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Replace the missing light shields above the rear counter food equipment.
The hose running out of the sanitizing solution dispenser above the three compartment sink is spraying liquid all over, repair the plumbing so water is contained.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. The light fixtures above the deli prep area have been taped up on the stainless steel and are full of dirty residue, remove the tape, clean off the residue and secure the lights in a sanitary manner.

The walls and ceilings shall be in good repair and easily cleaned.MUST CLEAN FOOD SPLASHES FROMCEILING TILES NEXT TO THE WALK IN COOLER.MUST REPAIR BROKEN WALL TILE AT WALLBASE BEHIND COCA-COLA REFRIGERATOR TO BE SMOOTH AND EASILY CLEANABLE.
