The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR TILES ARE BROKEN BY THE SECOND HAND SINK IN BACK PREP AREA. INSTRUCTED TO REPLACE THE BROKEN TILES.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. SECOND HAND SINK IN THE BACK PREP AREA WATER GOES DOWN VERY SLOWLY. INSTRUCTED TO REPAIR SINK SO WATER GOES DOWN PROPERLY.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. CLEAN THE LIGHT SHIELDS IN THE WALKIN COOLER AND WALKIN FREEZER.

THE TABLE TOP AND SNEEZE GUARD FRAME IN FRONT OF THE HOT CHICKEN HOLDING HAVE SOME FOOD BUILDUP IN THE CORNERS; CLEAN, RINSE AND SANITIZE AREA IN DETAIL.
THE CABINET BELOW THE SODA DISPENSER IS WET AND THE AREA BEHIND THE ICE DISPENSER IS DIRTY; CLEAN, RINSE, SANITIZE AND DRY SAID AREAS.
