FOUND EVIDENCE OF RODENTS ON SITE. OBSERVED APPROXIMATELY 30-35 MICE DROPPINGS SCATTERED UNDER FRONT PREP LINE COOLER,IN CABINET UNDER PREP LINE COOLER,UNDER STORAGE SHELVES IN REAR AND ALONG WALLS. MUST REMOVE DROPPINGS,SANITIZE AFFECTED AREAS AND HAVE PEST CONTROL COMPANY SERVICE AREAS. SERIOUS CITATION ISSUED 7-38-020.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. MUST DETAIL CLEAN INTERIOR OF ICE MACHINE AND REMOVE RUST. ALSO DETAIL CLEAN INTERIOR OF MICROWAVE,CUTTING BOARDS,ALL PREP TABLES,COOLERS AND INTERIOR OF CABINETS AND REMOVE FOOD DEBRIS AND MICE DROPPINGS. MUST ALSO DETAIL CLEAN TOILET AND WASH BOWL IN CUSTOMER WASHROOM.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR BROKEN VENTILATION IN EMPLOYEE WASHROOM. ALSO CLEAN VENT COVER IN CUSTOMER WASHROOM AND REMOVE FOUL ODORS. MUST REPAIR LEAKY PIPES UNDER 3 COMPARTMENT SINK.

When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code. NO HOT WATER AT SINK IN CUSTOMER WASHROOM. INSTRUCTED MANAGER TO PROVIDE HOT WATER AT WASHBOWL FOR PROPER HAND WASHING. SERIOUS VIOLATION 7-38-030 CITATION ISSUED.
