All utensils shall be thoroughly cleaned and sanitized after each usage. MUST CLEAN-SANITIZE; SMALL REACH IN COOLER AT FRONT SERVING AREA, MICROWAVE OVENS, CABINET UNDER EXPOSED SINK FRONT AREA AND CABINET UNDER SODA MACHINES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST CLEAN FLOORS UNDER 3PART SINK AND THRU-OUT REAR STORAGE AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKING FAUCET AT 3PART SINK.
MUST REMOVE UNNECESSARY ARTICLES THRU-OUT DISH WASHING AREA, CLEAN AND ORGANIZE TO PREVENT PEST HARBORAGE. ARTICLES MUST BE STORED OFF FLOORS, AWAY FROM WALLS.

Found 3 compartment sink not draining properly, 2nd of 3 compartment sink backing up not draining properly, must be repair. Citation#H000072737 issued with a court date of 11/17/2011, at 400 w Superior in Room #112 at 2:00 PM.
Walk in cooler shelving that had peeling paint rust, shall be replaced/repaired. Reach in cooler shelving that had peeling paint/rust shall be replaced/repaired.
Non food contact surfaces of walk in cooler shelving/racks, rear storage shelving, and reach in cooler not clean need detailed cleaning(crevices).
Floors in walk in cooler not clean need detailed cleaning under shelving racks, floors in rear storage areas under shelving not clean need detailed cleaning(corners).
Walls in rear prep area by 3 compartment sink, and prep table not clean need cleaning.

INSTRUCTED TO PROVIDE A WASHABLE SPLASH GUARD AT THE REAR PREP HAND WASH SINK BETWEEN SINK AND FOOD STORAGE SHELVES.
NOTED LEAKAGE AT THE U-JOINT OF HAND WASH SINK AT THE REAR FOOD PREP AREA. MUST REPAIR AND MAINTAIN.
MUST PROVIDE AN ACCURATE THERMOMETER INSIDE THE WALK IN COOLER AND WHERE EVER NEEDED FOR PROPER TEMPERAURE MONITORING.
