All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.OBSERVED THE FOLLOWING NOT CLEAN EXTERIOR OF COOKING EQUIPMENT WOK, SIDE OF STOVE AND THE INTERIOR BASEMENT BOX FREEZER, 1ST FLOOR ICE MACHINE LID, AND REAR MICROWAVE, UPRIGHT FREEZER.ALSO OBSERVED THE STORAGE SHELF ABOVE SALAD COOLER NOT CLEAN INSTRUCTED TO CLEAN.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.OBSERVED THE FLOOR UNDER THE BASEMENT STEPS NOT CLEAN INSTRUCTED TO CLEAN.
The walls and ceilings shall be in good repair and easily cleaned.OBSERVED THE WALL NOT CLEAN ABOVE THE SALAD COOLER.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.OBSERVED 3 KNIFES IMPROPERLY STORED BETWEEN WALL AND 3 COMPARTMENT SINK. FOUND EVIDENCE OF MICE DROPPINGS INSIDE THE GARAGE BEING USED AS A STORAGE ROOM. ALONG THE WALL BASE ,BEHIND ALL THE COOLERS AND UNDER ALL THE CHAIRS AND STOVE, APPROXIMATELY OVER 85 TOTAL. INSTRUCTED TO CLEAN REMOVE THE DROPPINGS AND SANITIZE THE AFFECTED AREA. ALSO MUST SEAL THE GAP ON THE SIDE OF THE GARAGE DOOR WITH OVER AN INCH GAP. CITATION ISSUED SERIOUS .
MUST PROVIDE THERMOMETER IN ALL CHEST FREEZERS IN THE BASEMENT AND MUST BE VISIBLE.(NONE OF THE 6 FREEZERS IN THE BASEMENT HAS A THERMOMETER).
MUST PROVIDE A PLACE OR RACK FOR ALL THE CLOTHINGS AND SHOES OF THE EMPLOYEES. FOUND ALL THE CLOTHINGS AND SHOES STORED BY THE DRY FOODS AND CANNED FOODS SHELVING IN THE BASEMENT.
FINAL SANITIZING RINSE OF LOW TEMPERATURE DISH MACHINE WAS AT 0PPM. MECHANICAL DISHWASHING MACHINE SHOULD PROVIDE A FINAL SANITIZING RINSE OF 100PPM. CITATION ISSUED CRITICAL. MANAGER CALLED ECOLAB TECHNICIAN AND HE WAS ABLE TO FIX THE MACHINE. FINAL SANITIZING RINSE WAS AT 100PPM ALREADY.
RUBBER GASKET ALONG THE DOOR OF THE PREP COOLER RIPPED OFF. INSTRUCTED TO REPAIR AND MAINTAIN. CAULKING AROUND THE EXPOSED HAND SINK WORN OUT. MUST RE CAULK AND MAINTAIN. DOOR OF THE TWO CHEST FREEZERS (2 OUT OF 6 FREEZERS)WITH RUST AND MOLDY TYPE SUBSTANCE AROUND THE DOOR AND NOT IN GOOD REPAIR. INSTRUCTED TO REMOVE RUST CLEAN IN DETAIL AROUND THE DOOR OR REPLACE DOOR.
MUST SEAL OR PAINT ALL THE RAW WOODS AROUND THE WINDOW IN THE REAR KITCHEN AND RAW WOODS OF THE EXIT DOOR.

MUST CLEAN IN DETAIL THE HOOD, FILTER,PIPE CONNECTING TO THE HOOD,TO THE STOVE AND PIPE CONNECTING TO THE GREASE TRAP BOX, LIGHT SHIELD ABOVE THE PREP AREA KITCHEN, AND WALLS BEHIND THE 3 COMPT SINK AND MAINTAIN.

KITCHEN/PREP AREA EXPOSED SINK NOT ACCESSIBLE FOR PROPER HAND WASHING. OBSERVED A CONDIMENTS STORAGE SHELVING UNIT IN FRONT OF SINK, AND MULTI-USE UTENSILS INSIDE THE SINK. SINK HAD EXCESSIVE DRIED FOOD PARTICLES IN IT. INSTRUCTED EXPOSED SINK MUST BE ACCESSIVE FOR PROPER HAND WASHING AT ALL TIMES. CRITICAL CITATION ISSUED, 7-38-030.
OBSERVED POOR HYGIENIC PRACTICES; ONE FOOD HANDLER, TOUCHING FACE/NOSE AND RETURED TO PREP AREA TO HANDLE/PREPARED FOODS. ANOTHER FOOD HANDLER WASHED HIS HANDS INSIDE 3PART SINK, AND ANOTHER EMPLOYEE CAME INTO THE KITCHEN AND PUT ON GLOVES WITHOUT WASHING HER HAND. INSTRUCTED HANDS MUST BE WASHED AFTER ANY CONTAMINATING CONDITION WITH SOAP AND WARM WATER AT THE EXPOSED HAND WASHING SINK. THE WASHING OF HANDS INSIDE A UTENSIL-WASHING SINK IS PROHIBITED.
