All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed accumilated food debris on exterior of blast chiller and inside prep area rubber door gaskets. Must clean and maintain all unclean equipment.

OBSERVED LIVE COCKROACHES AROUND THE DISHWASH AREA. 4 LIVE COCKROACHES WERE OBSERVED ON THE WALL BEHIND THE DISHWASH MACHINE. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.

REPAIR THE LEAKING FAUCET AT THE 2 COMPARTMENT SINK AND THE DRAIN LINE WATER LEAK BELOW THE 3 COMPARTMENT POTWASH SINK.

QUALITY AND EXCELLENCE PEST CONTROL ONSITE TO MINIMIZE THE PEST ACTIVITY. THE BOILER ROOM WAS TREATED DURING THE INSPECTION AND ADDITIONAL FOOD SERVICE AREAS WILL BE TREATED SUBSEQUENTLY.
