Violation descriptions and comments

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. The main boy's toilet bowl is out of order, several alternate toilets are available while awaiting repair parts.
The walls and ceilings shall be in good repair and easily cleaned. Repair the fallen ceiling in the room 224 bathroom.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Wash, rinse and sanitize the 2 door reach in cooler interior and the cooler in room 222.

The single basin sink in the cafeteria is inadequate to rinse utensils being sent back to Andrew Jackson School and wash hands as well, (it has an attached drinking fountain nozzle). Water is leaking into the drawers of clean utensils from said sink while utensils are washed. Install a three compartment sink with an attached drainboard to wash, rinse and sanitize items in this school in a sanitary manner. Repair the North girls toilet. Repair the leaking playroom sink faucet.

The hand sink in pre-school room #4 has no hot water, after running for ten minutes it is 83.4 f and rusty, repair plumbing so the children's bath room is fully functional. The second wash room in pre-school room #4 is blocked off due to a damaged crumbling ceiling and an inoperative leaking toilet covered with a garbage bag. Repair the bathroom so it is fully functional or cap off and remove unsanitary plumbing. Adequate bathroom facilities are in the hallway.
The following violations were originally documented on January 5, 2011 and were given a correct by date of April 05, 2011 per inspection report # 120703. Clean and repair the water damaged kitchen cabinet where the sink is. A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority.

EMERGENCY WORK ORDER # 230193 DATED 11/17/2011 REQUESTING INSTALLATION OF A 3-COMPARTMENT SINK IN THE CAFETERIA WAS PUT IN AT THIS TIME. (COPY INCLUDED) CHIEF SANITARIAN AND DIRECTOR OF FOOD PROTECTION DIVISION CONSULTED AT THIS TIME. CHARTWELLS SUPERVISOR MS. ARRIVEE TULLIVER WAS CONTACTED AND AGREED TO SEND FRESH SANITIZED UTENSILS WITH THE FOOD DAILY AND TO COLLECT THE DIRTY UTENSILS DAILY UNTIL THE SINK CAN BE INSTALLED OVER THE CHRISTMAS BREAK. IF THE WORK IS NOT COMPLETED BY THE START OF SCHOOL IN JANUARY 2012 THE SCHOOL WILL BE PUT ON ALTERNATIVE FOOD SOURCE BOX LUNCHES PER THE CHICAGO DEPARTMENT OF PUBLIC HEALTH.
