ON 7-7-2010 LICENSE INSPECTION WAS DONE AND APPROVED WITH EXPOSED HAND SINK IN PREP AREA,TODAY EXPOSED HAND SINK IS LOCATED IN HALLWAY NEXT TO WASHROOM DUE TO OWNER BUILD A WALL AFTER RETAIL LICENSE WAS APPROVED BY PUBLIC HEALTH; OBSERVED EMPLOYEE WASHING HAND ON SET UP 3 COMPARTMENT SINK.INSTRUCTED TO REMOVE THE WALL OR RELOCATE THE EXPOSED HAND SINK WERE ORIGINALLY WAS APPROVED.(IN PREP AREA).SERIOUS VIOLATION:7-38-030 H000072169-18

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. KNIFE RACK IS NEEDED AT SUSHI AREA.DO NOT STORE KNIFE ON TOP THE REFRIGERATOR.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
