THE FOLLOWING POTENTIALLY HAZARDOUS FOODS FOUND HELD AT IMPROPER TEMPERATURES. APPROXIMATELY 1LB SHRIMP IN THE ABOVE MENTIONED COOLER AT 45.5F, 8LBS BEEF AT 45.9F, 6LBS CHICKEN AT 49.5F AND 3LBS CUT ASSORTED VEGETABLES. ALSO, BLACK BEANS IN HOT HOLDING STEAM TABLE AT 96.2F, CHEESE SAUCE AT 72.1F. INSTRUCTED MUST REHEAT ALL PREPARED FOODS RAPIDLY TO 165F THEN HOLD HOT IN STEAM TABLE AT A MINIMUM OF 140F. NO VALID CITY OF CHICAGO SANITATION CERTIFICATE POSTED ON PREMISES OR PERSON ON PREMISES WHILE OPERATING AND PREPARING FOODS.

MUST REPLACE/REPAIR.FLOOR BASEBOARDS THROUGHOUT REAR LOWER LEVEL AND EMPLOYEE WASHROOM MISSING WITH LARGE HOLES, OPEN CREVICES. MUST SEAL/REPAIR. MISSING FLOOR DRAIN COVER IN REAR PREP, MUST PROVIDE.
WATER DAMAGED CEILING TILES IN EMPLOYEE WASHROOM AND REAR PREP. MUST REPLACE. WALL IN EMPLOYEE WASHROOM AROUND SINK DAMAGED WITH OPEN CRACKS/CREVICES, MUST REPAIR/SEAL.
MUST REMOVE DEAD INSECTS FROM LIGHT SHIELD ABOVE WALK-IN COOLER.
ALL UNUSED EQUIPMENT THROUGHOUT REAR MUST BE REMOVED TO PREVENT RODENT AND INSECT HARBORAGE.
ALL GUN HOLDERS BEHIND BAR MUST PROPERLY DRAIN TO FLOOR DRAIN. MUST PROVIDE VENTILATION IN BOTH MENS AND WOMANS WASHROOM.
MUST PROVIDE PROPER ICE SCOOPS WITH HANDLES BEHIND BAR, NOT METAL MIXER JARS.
OBSERVED THE FOLLOWING DRAIN FLIES IN THE FOLLOWING AREAS, APPROXIMATELY 6 AT REAR DISH AREA, 9 IN EMPLOYEE WASHROOM, OVER 75 THROUGHOUT REAR LOWER LEVEL STORAGE AREA AT WALK-IN COOLER AND ENTRANCE TO OFFICE. INSTRUCTED TO HAVE PEST CONTROL CO. SERVICE FOR FLIES TO MINIMIZE OR ELIMINATE FLY ACTIVITY AND SANITIZE ALL AFFECTED AREAS.
