THE FOLLOWING NOT CLEAN- OBSERVED SLIGHT LIME BUILD UP INSIDE ICE MACHINE, DUST/FOOD DEBRIS ON SHELVES UNDER STEAMTABLE AT GRILL PREP AREA. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED FOOD/DIRT BUILD UP INSIDE FLOOR DRAIN UNDER 2-COMPARTMENT SINK. MUST CLEAN AND MAINTAIN INTERIOR OF ALL UNCLEAN FLOOR DRAINS.

SINK IN DISHROOM; 10-ON WALLS & CEILING OF A STORAGE ROOM ALONGSIDE THE SELF-SERVE SODA STATION. INSTRUCTED TO ELIMINATE SOURCE & CLEAN/SANITIZE ALL AFFECTED AREAS. All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.
ALL CUTTING BOARDS WITH DEEP/DARK GROOVES MUST BE RELACED. THE GREASE TRAP UNDER THE 3-COMP. SINK HAS EVIDENCE OF CORROSION AROUND THE BASE-MUST REPLACE. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
FLOORS ALONG WALLBASES UNDER SINKS & HEAVY EQUIPMENT IN MAIN KITCHEN & DISHROOM, INCLUDING FLOOR DRAINS UNDER EQUIPMENT MUST BE KEPT CLEAN. FLOORS IN DISHROOM MUST KEPT DRY TO PREVENT PEST HARBORAGE. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
A LEAK FROM A WATER LINE BEHIND THE STEAMER MUST BE FIXED. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
WALLS OR CEILING TILES AFFECTED BY PESTS MUST BE CLEANED/SANITIZED. The walls and ceilings shall be in good repair and easily cleaned.
