All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin/insect infestations. Found evidence of mice activity on premises. Noted approx 50 mice droppings on cabinets under front serving line; approx 50 droppings behind equipment under front counters. Must clean /sanitize entire premises & contact P.C.O. Citation issued ( per municipal code/rules & regs). Also noted 2-live mice on a glueboard around soda lines.
Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. Observed employee washing,rinsing equipment at 3-comp sink, by running them under faucet water, without properly setting up for wash,rinse,sanitize.
All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. Observed the middle, bottom pipe of the 3-comp sink leaking profusely onto the floors.Premises using red bucket to hold up sink. Must fix to prevent pest issues. Citation issued per city code/rules & regs.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors thruout premises especially along wallbases, including the attic area must be kept clean/sanitized. Must clean floors of walk in cooler also.

Must replace fiberboard shelvings at soda station, under microwave oven, that are in poor repair. Must replace caulking or seal around sink drain boards also in poor repair.

Must properly rodent proof the rear door to hallway due to about 2-inches gap at bottom of door. Also found evidence of small/fruit fly activity on walls around 3-comp sink approx 20 found.

Front handsink faucets are leaking & not properly maintained. Must replace fix/knobs.

Over 30 mouse droppings AND DEAD COCKROACHES were found around and under shelving and baseboards throughout the 2ND FLOOR OFFICE/STORAGE AREA. Remove all evidence of pest activity and clean up all areas of possible harborage.

THE RACKS IN SAID COOLER ARE CORRRODED AND ALSO FULL OF FOOD BUILDUP, WASH, RINSE AND SANITIZE THE COOLER INTERIOR AND RACKS AND REPAIR THE WALL SO IT IS SMOOTH AND SANITARY.
