HAS A LARGE AMOUNT CLUTTER - WOOD AND METAL IN THE WEST SIDE OF THE BOILER ROOM. INSTRUCTED TO REMOVE THE CLUTTER AND CLEAN AND ORGNIZE SO NO INSECT OR RODENT HARBORAGE.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. The walk in cooler threshold is dirty; wash, rinse and sanitize the threshold area of the walk in cooler.
The three compartment sink has a potable water leak in the right side hot water tap, repair said leak. The second floor boy's washroom has an out of order urinal (one of six), repair the urinal.
Cleaning equipment shall be properly stored away from food, utensils, equipment, and clean linens. Bottles and cans of floor cleaner and oven cleaner are on the three compartment sink wash compartment drain board. Store the floor and oven cleaner away from the three compartment sink.
