The light shields are dirty and damaged or missing above the main hot line in the front kitchen, clean off the lighting and repair or replace the shields so there is a barrier between the glass bulbs and the food preparation area.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. The north walk in cooler has some ice buildup on the coil and the door has a piece of metal keeping the door from closing properly, repair the cooler door and defrost the coil.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. The cutting boards are stained, the can opener is dirty and the blade is damaged; wash, rinse, sanitize and repair the cutting boards and can opener so they are in sanitary condition.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. The floor tiles are missing and damaged under the meat grinder and the base boards are in poor repair with large gaps where food and water can get trapped, clean and repair the floors and base boards so they are smooth, sanitary and easy to maintain.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. The clean food pans on the rear drying rack need to be inverted so they can properly air dry.

THE BREAKFAST ITEMS IN THE EGG STATION AND THE MAIN KITCHEN SERVING LINE HAVE NO PREPARATION COOLERS, POTENTIALLY HAZARDOUS FOODS INCLUDING PRE-COOKED CHORIZO SAUSAGE, SHREDDED BEEF AND SHELL EGGS, CHEESES, SHREDDED CHICKEN, DICED TOMATOES, CHOPPED LETTUCE IS STORED ON UNDRAINED ICE. ADEQUATE MECHANICAL REFRIGERATION MUST BE INSTALLED IN ORDER TO KEEP FOODS AT 40 F OR LESS IN A SANITARY MANNER EASILY ACCESSABLE TO THE KITCHEN SERVING LINE. FOOD NOT DISCARDED PER VIOLATION #03 IS BEING MOVED TO THE 3-DOOR COOLER ACROSS THE SERVING LINE OR WALK IN COOLERS IN THE REAR PREP AREA.
APPROXIMATELY 20 POUNDS OF POTENTIALLY HAZARDOUS FOODS INCLUDING PRE-COOKED CHORIZO SAUSAGE, SHREDDED BEEF, CHEESES AND SHREDDED CHICKEN IS STORED ON UNDRAINED ICE AT 57-69 F. APPROXIMATELY $60.00 WORTH OF PRODUCT WAS DISCARDED AT THIS TIME. POTENTIALLY HAZARDOUS FOODS MUST BE HELD AT 40 F OR LESS.
OVER 25 LIVE COCKROACHES WERE FOUND IN THE BASEMENT AND BASEMENT STAIRWELL IN GAPS IN THE DRY STORE ROOM DOORWAY, WATER DAMAGED WALLS AND CEILINGS AND OPEN DRAINS. OVER 30 LIVE FLIES AND DRAIN FLIES WERE FOUND IN THE BASEMENT AND SCATTERED THROUGHOUT THE ESTABLISHMENT. REMOVE ALL EVIDENCE OF PESTS AND CONSULT WITH ORKIN PEST CONTROL AS THEY ARE DOCUMENTING NUMEROUS SIGHTINGS AND SOLUTIONS THAT NEED TO BE IMPLEMENTED IN ORDER TO REDUCE HARBORAGE.
