Found/Observed rodent droppings(300+) throughout premises, in rear kitchen prep areas, under prep tables, storage shelving, heavy equipment(coolers), rear storage areas on storage shelving, unused equipment, bathroom, front counter shelving.Also found front entrance/exit door not rodent proofed, door had 1/4 inch gap on middle of doors, rear exit door needs adjustment to properly close. Critical violation, citation issued. 7-38-020. Must remove all droppings, wash and sanitize all surfaces, have premises serviced by pest control company and correct all following violations.
Mold and slime build-up on interior surfaces of dairy cooler in rear. Must wash, sanitize and maintain equipment. Citation issued, serious violation. 7-38-005a.
Found observed cooking equipment, prep tables, cooler shelving, storage shelving with excess grease, food debris accumulation throughout kitchen equipment, needs detailed cleaning (excess food bebris, grease removed, and sanitized).Rear storage shelving not clean need detailed cleaning(crevices), stove need extra detail cleaning(oven), front display cooler door tracks, shelving need detailed cleaning and sanitizing. Front counter shelving and dining room chairs and tables needs cleaning.
Exposed raw wood on front counter(shelving), rear areas, and by exposed hand sink, shall be repair and sealed to be non-absorbant. Cooking pots too large for 3 compartment sink. Must remove over-sized equipment/utensils from premises.
Floors throughout premises not clean had excess food debris, food particles, grease build up in kitchen prep areas. Floors need throughot cleaning, and sanitizing throughout premises especially under heavy equipment, rear kitchen, and storage areas(corners, and along wallbase). Dead mouse on floor in rear storage area.
Walls in kitchen prep areas need detailed cleaning and sanitizing, peeling paint on walls in kitchen areas ,rear storage areas shall be repair. Repair bent wall splashguard above 3 compartment sink. Clean ceiling/wall vents through-out.
Missing light shields in prep and service areas, shall be provided, light shields that were missing end caps shall be provided, coolers that had missing,damaged light shields shall be repair/replaced. Some lights not working in rear storage areas, must have working light fixtures.
Exhaust vents(ventilation) in kitchen prep area, coolers, and bathroom need cleaning. Exhaust canopy not clean, needs detailed cleaning. Repair and maintain plumbing under mop sink. Provide working drain plugs (3) for 3 compartment sink. All cooking equipment must be under ventilation hood.
Excess debris and clutter in rear storage areas, kitchen areas shall be removed. Stock items must be stored six inches off of floor to prevent pest harborage.
Coolers missing conspicuously placed thermometers, also no metal stem thermometer. Must provide and maintain thermometers.
