All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.OBSERVED THE MICROWAVE TRAY IN POOR REPAIR INSTRUCTED TO REPLACE.
The walls and ceilings shall be in good repair and easily cleaned.OBSERVED A BLACK SUBSTANCE ON THE CEILING PIPE ALSO WATER DROPLETS ON THE SAME PIPE INSTRUCTED TO CLEAN AND DRY PIPE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.OBSERVED STANDING WATER IN THE BASEMENT FOLLOWING AREAS BEHIND WASHER, FRONT HOT WATER TANK, MULTI PLEX MACHINE. INSTRUCTED TO REMOVE STANDING WATER. ALSO THE FOLLOWING AREAS NOT CLEAN UNDER BAG IN BOX STORAGE, FIRST FLOOR UNDER FRYERS, AND COOKING STOVE.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.OBSERVED THE LIGHT SHIELDS NOT CLEAN IN THE FOLLOWING AREAS DRIVE THRU , OFFICE. INSTRUCTED TO CLEAN.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.OBSERVED WATER LEAKING FROM HOSE CONNECTION AT THE HOT WATER TANK INSTRUCTED TO REPAIR.

INSTRUCTED TO CLEAN/SANITIZE; FRYERS, MOP SINKS, AND EXTERIOR OF FROZEN DESERTS MACHINES.
MUST CLEAN FLOORS UNDER EQUIPMENT AT PREP AREAS, INSIDE MOP CLOSETS AND THRU-OUT BASEMENT AT CORNERS AND ALONG WALLS.
INSTRUCTES TO CLEAN CEILING ABOVE HOT LINE/PREP AREA AND HOOD AND FILTERS ABOVE FRYERS.
