The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPLACE DAMAGED FLOOR TILES IN HALLWAY AND WASHROOM AREAS. CLEAN FLOORS UNDER ALL EQUIPMENT ALONG WALLS AND IN ALL CORNERS IN FURNACE ROOM, CLASSROOM, PREP AND STORAGE AREAS.
The walls and ceilings shall be in good repair and easily cleaned. SEAL ALL OPENINGS ALONG WEST WALL BEHIND UTILITY SINK WHERE WALL AND FLOOR MEET, ALONG BASEBOARDS IN CLASSROOMS. WALL NEED REPAIR IN SOUTHEAST CORNER BEHIND CUBBIES NEAR FRONT WINDOW. SEAL ALL OPENINGS IN WALLS AND CEILINGS, AROUND PIPE FITTINGS, ELECTRICAL OUTLETS, ETC IN ALL AREAS. STORE ALL EQUIPMENT AWAY FROM WALLS FOR EASY CLEANING AND PEST CONTROL. WALLS NEED CLEANING IN ALL CORNERS IN CLASSROOMS (DUST BUILDUP, WEBS, ETC). INSTALL SMALLER SCREEN ON VENT IN FURNACE ROOM. REPLACE ALL STAINED AND DAMAGED CEILING TILES. CLEAN ALL CEILING VENTS.

FOUND APPROXIMATELY 25 MICE DROPPINGS SCATTERED IN FRONT CLASSROOM IN SOUTHEAT CORNER,ON TOP OF PLAY TABLE IN REAR UNUSED CLASSROOM,ALONG SIDE THE STOVE AND REFRIGERATOR AND INSIDE THE FOAM PLATFORM UNDERNEATH THE REFRIGERATOR. MUST REMOVE DROPPINGS,SANITIZE AFFECTED AREAS AND HAVE A PEST CONTROL COMPANY SERVICE AREAS. ALSO, THE PEST CONTROL LOG BOOK ON SITE IS INCOMPLETE AND CONTAINS DOCUMENTS DATED BETWEEN 2004-2006. MUST HAVE A PEST CONTROL COMPANY PROVIDE A COMPLETE PEST CONTROL LOG BOOK CONTAINING ALL REQUIRED DOCUMENTS AND PREMISES MUST PROPERLY MAINTAIN LOG. SERIOUS CITATION ISSUED 7-38-020.
